Job SummaryPosition Title: Health Liaison Program: Thrive to Five Program Classification: Permanent/Full-Time/Hourly DUTIES AND RESPONSIBITIESProgram Health
Sensory/Health Screenings – Responsibility role for collaborating with Health Specialist to complete Vision, Hearing, Dental, and Growth Screenings.
Utilize partnerships between program leaders, staff, and outside agencies to coordinate, schedule, and conduct necessary health screenings within mandated time frame requirements.
Assist with monitoring to ensure screening cycles are completed and properly communicated.
Assist with data entry compliance and keeping information on health partners up to date for referrals.
Child and Family Health Services
Assists in the implementation of a tiered intervention system for meeting family/child health needs: check-ups, immunizations, establishing a medical home, obtaining health insurance, following up on screening referrals, and chronic health and nutrition concerns.
Connect with families to assist with establishing medical and dental appointments as needed offering additional support when referred by the Family Liaison.
Collaborate on developing program plans to address education and interventions for community health needs as identified by assessments such as parent surveys
Data and Record Keeping
Monitor and report on program compliance in delivering health services.
Maintain current health records of children. Includes saving and entering health records into the appropriate database.
Collaborate on monitoring of information and address barriers and hurdles with staff and families.
Follow communication systems to alert appropriate staff to information/records needed, or changes that occur.
Program Health Support
Support program staff in knowledge of medical information and processes around current databases.
Collaborate with Health Specialist and Coordinator of Supports to access and/or create resource materials for staff and families around health, safety, nutrition, mental health, and disabilities topics as needed.
Education/Experience Requirements
Bachelor’s degree in Health, Education, or Social Work fields OR Associate degree in Health, Education, or Social Work fields with 2 years’ experience.
