Community Action Partnerships of Western Nebraska (CAPWN) is a non-profit community-based health and human services organization that serves low-income, disadvantaged and those unable to meet their needs through other sources. The agency was established in 1965 and provides services to over 9,000 individuals, children and families in three main areas:
  • Community Health Services: WIC, Commodity Supplemental Food, Foster Grandparents, Migrant Seasonal Head Start, and Child Development Center.

Youth Rapid Rehousing Case Manager

Job Title
: Youth Rapid Rehousing Case Manager
Reports To: Housing First Manager

Areas of Responsibility
: Under the supervision of the Housing First Manager, this position is responsible for the implementation and delivery of comprehensive case management and advocacy services for individual youth people ages 18-24, and their families participating in the Rapid Re-Housing Program. This position provides services based on the Housing First Model of care with motivational interviewing, strengths-based goal setting, and trauma-informed care approach to work collaboratively with individuals and families towards the general goals of maintaining housing, increasing family stability, in providing family functioning and increasing the self-sufficiency in the program.

Major Duties
:
  • Conduct the VI-SPDAT with youth who are experiencing homelessness.
  • Receive referrals from community queue and verify household meets criteria for program.
  • Intake and assessment of referred individuals/families for Rapid Re-Housing Program.
  • Maintain updated knowledge and offer youth emotional support, information about community resources and services (including employment, housing, shelter services, health care, etc.), and assistance completing paperwork (social services applications, legal filings, etc.).
  • Maintain a caseload of approximately 10-15 youth.
  • Work to secure rental agreements with landlords and assure compliance with HUD requirements.
  • Conduct ongoing, individualized case-management to youth in the program.
  • Assist youth in developing and implementing an individualized program plan to meet long-term housing goals.
  • Develop and facilitate support, informational classes/sessions, life skills, and/or group activities designed to help clients explore and resolve self-care issues that may be influencing the clients’ progress towards meeting their established goals.
  • Maintain accurate and confidential case files.
  • Do statistical data entry, complete routine paperwork and produce small reports.
  • Develop relationships with landlords and property management companies.
  • Attend and participate in staff meetings and trainings as requested.

Working Conditions:
  • On-site work 40%
  • Off-site home visits, training, meetings, community interactions 60%
  • Work requires ability to meet tight timelines.
  • Work receives limited supervision and requires the ability to take initiative and be self–directed.
  • Occasional out-of-town travel for training.

Physical Requirements:
  • Must be able to move about quickly if necessary.
  • Must be able to lift 20 lbs.
  • Physically able to perform job.

Minimum Qualifications
:

Grade: 12
  • High School education or equivalent
  • One year of documented work experience in a related field
  • Experience with dual-focus shelter programs homeless shelters, substance abuse programs, social services programs, preferred.
  • Knowledge of “Housing First” approach to end homelessness, preferred.
  • Knowledge or understanding of fair housing, tenant/landlord rights and responsibilities, preferred.
  • Ability to communicate effectively both orally and in writing and good organizational skills.
  • Able to attend meetings and training sessions in and out of area.
  • Must have a valid driver’s license with good driving record and insured vehicle available at all times.
  • Age 21 or above
  • Must pass child/adult abuse, criminal history and sex offenders background check.

Grade: 13
  • Must meet minimum requirements above.
  • AA Degree in a related field, or two years’ experience

Grade: 14
  • Must meet minimum requirements above.
  • AA Degree in a related field plus two years' experience, or BA Degree in a related field or comparable combination of education and experience
