CommuniCare Family of Companies is Hiring a Primary Care Nurse Practitioner Near Blue, OH

CommuniCare Health Services, in conjunction with our Medical Practice, is currently recruiting a Primary Care Nurse Practitioner.

THE ROLE

The Primary Care Nurse Practioner is responsible for assisting with implementation of strategic network initiatives, supporting growth of our innovative healthcare practice, and providing high quality, value based primary care to the senior patient population within our network. Our network consists of a variety of payors, an accountable care organization (ACO), telemedicine, and Medicare advantage products. The Nurse Practitioner is instrumental in assisting our Primary Care Physician in our person-centered care model in delivering quality care with smaller panels and more frequent touches. The Nurse Practitioner will participate in streamlining healthcare collaboration of the interdisciplinary team, while driving high quality clinical outcomes. This approach will reduce hospital and emergency room department admissions and visits, focus on preventive medicine, and lead a culture focused on customer experience and satisfaction.

PRIMARY RESPONSIBILITIES AND ESSENTIAL FUNCTIONS:

  • Assess and diagnose patients
  • Develop treatment plans and verbalize those to patients effectively
  • Collaborate with PCP on initiatives
  • Assist with care coordination with other providers and specialists
  • Flexible to work in multiple healthcare settings such as clinics, assisted living, independent living, and home care
  • Administer primary care and health education to patients in our network
  • Oversee clinical coordinator and/or other clinical care coordination team members
  • Complete annual wellness visits and/or health risk assessments
  • Support strategic initiatives regarding value-based care
  • Collaborate with interdisciplinary clinical team
  • Maintain appropriate and compliant documentation within our EMR (electronic medical record) software
  • Collaborate with local and regional operations and clinical leadership to achieve goals
  • Provide an excellent customer experience to foster high customer satisfaction/retention
  • Assist the PCP of their assigned practice to foster growth of the practice and deliver positive clinical outcomes and experiences
  • Follow all state and federal regulations, guidelines, and laws
  • Other duties as assigned.

EDUCATION and QUALIFICATION:

Required:

  • Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
  • National certification of AANP or ANCC
  • Active, unrestricted state license
  • Active DEA license
  • US work authorization
  • 2 years in primary care or family medicine
  • Ability to work in a flexible environment and schedule
  • Passionate about providing healthcare to the senior and low-income populations

Preferred Qualifications:

  • Training or clinical experience in geriatrics
  • Experience with coding, ICD-10 and HCC (Hierarchical Condition Categories)
  • Experience or training working in a metrics, results driven healthcare model
  • Be open to professional development through training, obtaining certifications if necessary, and attending team meetings
  • Experience working in value-based care models
  • Experience working in a collaborative healthcare setting to drive positive outcomes and achieve goals
  • Training or experience in outpatient primary care setting

JOB SKILLS:

  • Must have strong oral and written communication skills
  • Must be detail oriented with an ability to work well both independently and in a team setting
  • Exhibit a customer service approach with teams and patients
  • Strong time management skills required. Must be able to prioritize and adhere to competing deadlines while achieving goals
