Nursing Scheduler – Home Health

Dignity Care is currently recruiting a Nursing Scheduler for our Home Health division.

What You Will Do as a Home Health Nursing Scheduler
As a Nursing Scheduler, you will assist the organization in achieving a centralized, efficient scheduling protocol of its multi-channel Home Health resources that will optimize patient care, resource optimization, and coordination of service offerings. Some of your primary responsibilities include:

• Meeting all service standards for exceptional customer service delivery
• Communicating with clinicians through case management entries and coordinating the processing of clinical documentation
• Confirming accuracy and entering time and travel in the database as applicable
• Scheduling clinicians to patients by using geographical origin, disciplines, caseload, and patient preferences
• Monitoring compliance with Home Health Aide supervision visits and functional reassessments
• Preparing daily/monthly/quarterly reports and audits as requested
• Ensuring confidentiality of financial and medical records in compliance HIPAA

Education, Licensure & Certification Requirements

• Three (3) years of related health experience, including working with scheduling 
• Previous customer service experience preferred
• Proficiency with computers; prefer experience with Microsoft Office including Word, Excel, and other software programs
