Due to current growth, we are recruiting for a Home Health Administrator for our Dignity Home Health Division located in Ohio. The position of Home Health Administrator will conceptualize how the division will scale and the preferred candidate will be entrepreneurial minded in order to perform the administrative functions of the role while leading a home healthcare team at the beginning of this region's launch. The position is based at our corporate office in Blue Ash (suburb of Cincinnati) Ohio.
Purpose of Position
The Home Health Administrator oversees patient care personnel and ensures the appropriate care coordination among all disciplines, physicians, and caregivers. The ideal Home Health Administrator will have at least 3 years of home health experience and a proven track record of leadership and managerial skills.

Primary Job Responsibilities
·Supervises the planning, coordination, and delivery of patient/family care.
·Manages clinical activities, including patient assessments, care plan development, service level determination, on-site field visits, and in relevant medical environments.
·Implementation/coordination/maintenance/evaluation of all care plans.
·Responsible for direct supervision of clinical staff while evaluating their performance relative to job goals/requirements.
·Participates in the recruitment, interviewing, selection, and orientation of team members.
·Coach staff, recommend in-service education programs, and ensure adherence to internal policies/standards.
·Ensure service quality and participate in care coordination to ensure proper communication between caregivers, patients, referral sources, and payers.
·Discusses operational issues by updating staff on new/changed regulations, and
·review records/documentation to ensure regulatory and in-house compliance.
·Be accountable for financial responsibility related budget goals through utilization, documentation, providing appropriate and accurate patient care, case mix weight, and appropriate utilization of delivery of patient care.
·Oversees the assessment and reassessment of patients, including updating care plans and interpreting patient needs, while adhering to company, physician, and/or health facility procedures/policies.
·Coordinate communications between team members/attending physicians/caregivers to ensure the appropriateness of care and outcome planning and ensure compliance by reviewing documentation and care coordination activities.
·Participate in performance improvement activities, maintain ongoing clinical knowledge through internal/external training programs, and provide interpretation of knowledge and direction to staff.
·Planning, directing, and evaluating operations to ensure the provision of adequate and appropriate care and services.
·Complying with applicable law and regulations
·Implementing Governing Body directives and ensuring that appropriate service policies are developed and implemented
·Recruiting, employing, and retaining qualified personnel to maintain appropriate staffing levels.
·Directing and monitoring agency performance improvement activities.
·Ensuring staff development including orientation, in-service education, continuing education, and evaluation of staff.
·Maintains standard of care related to State, Federal, OSHA, and CHAP guidelines.
·Participates in appropriate team conferences, staff meetings, in-service, and committees with physicians and agency staff to ensure continuity of care.
·Completes and submits all documentation, including clinical and progress notes in a timely manner according to agency policy working with Account Executives.
·Oversight for operations, clinical excellence, and growth differentiation in service areas.
·Explain and mentor clinicians and staff on COPs and on billed skill visits.
Education and Qualifications:
·Minimum of 3 years of home care experience.
·Knowledge of state and federal home health agency regulations/conditions of participation and compliance standards and regulations.
·Proven track record of clinical management, education, and management of healthcare staff
·Knowledge of clinical policies and procedures and ability to implement good organization, communication, human relations skills, and reliable transportation.
·Must possess the ability to lead and manage clinicians and office staff for successful patient outcomes and clinical and branch performance.
·Licensed Registered Nurse and active BLS preferred.
