CommuniCare Family of Companies is Hiring a Director of Clinical Operations and Program Integration Near Cincinnati, OH

Director of Clinical Operations and Program Integration (MSN, DNP, or NP)

PHP Director of Clinical Operations & Program Integration has a passion for overseeing the delivery of quality care to seniors and other residents in a variety of healthcare settings that can include, but are not limited to, skilled nursing facilities, assisted living, and independent living communities, as well as via telehealth.

Job Summary:

The Personalized Health Partners (PHP) Director of Clinical Operations and Program Integration is responsible for overseeing the high-quality primary care in a high touch, person-centered care model with a special focus of supervising clinical requirements associated with special needs plans (i.e.: ISNP, ACO). The PHP Director of Clinical Operations and Program Integration will provide oversight of the management of clinical operations within PHP. The PHP Director of Clinical Operations and Program Integration will manage staff and resources, including hiring, training, performance management and evaluation.

PRIMARY RESPONSIBILITIES AND ESSENTIAL FUNCTIONS

  • Ensure the delivery of high-quality patient care and compliance with regulations and standards
  • Provide ongoing performance feedback throughout the year, coach employees in disciplinary matters, and obtain assistance from VP of Clinical Operations for disciplinary actions and/or termination.
  • Identify educational needs and work with educators to develop and implement training
  • Develop and implement operational strategies for clinical team
  • Review clinical opportunities and recommend strategic plans to improve them
  • Maintain appropriate and compliant documentation within our EMR (electronic medical record) software
  • Develop relationships with key stakeholders associated with special needs plans
  • Support strategic initiatives regarding value-based care
  • Provide an excellent customer experience to foster high customer satisfaction/retention
  • Practice ethically and in accordance with the Scope and Standards of Practice of their profession and Board Certification.
  • Follow all state and federal regulations, guidelines, and laws 

Additional duties of DCOs

  • Collaborate with the leadership teams overseeing ACO and special needs plans
  • The Director of Clinical Operations and Program Integration will complete at least quarterly in person 1:1 visit with assessment NPs
  • The Director of Clinical Operations and Program Integration role will be a 40% field-based and 60% administrative based position
  • The Director of Clinical Operations and Program Integration will provide weekly performance reviews to NP’s regarding productivity, completion rate of biannual HRAs and AWVs, unsigned notes, cancelled visits and uncompleted Relias trainings. 

EDUCATION and QUALIFICATION:

Required:

  • Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
  • National certification of AANP or ANCC
  • Active, unrestricted state license/s
  • Ability to obtain licensure in additional states in service area as requested (telehealth providers required to be licensed in OH, IN, WV, MD, VA, PA, MO)
  • Active DEA license
  • US work authorization
  • 2 years clinical experience as a Nurse or Nurse Practitioner in geriatric, adult or family practice setting
  • Ability to work in a flexible environment and schedule
  • Passionate about providing healthcare to the senior and low-income populations

Preferred Qualifications:

  • 1 years clinical management experience as a Nurse or Nurse Practitioner in geriatric, adult or family practice setting
  • 2 years clinical experience in special needs plans
  • Training or clinical experience in geriatrics
  • 3 years clinical experience as a Nurse Practitioner
  • Experience with coding, ICD-10 and HCC (Hierarchical Condition Categories)
  • Experience or training working in a metrics, results driven healthcare model
  • Be open to professional development through training, obtaining certifications if necessary, and attending team meetings
  • Experience working in value-based care models
  • Training or experience in geriatric, assisted living, and/or skilled nursing setting
  • Experience working in a collaborative healthcare setting to drive positive outcomes and achieve goals
  • Training or experience in outpatient primary care setting
  • Experience with excel, reporting and understanding of EMR systems
