Commonwealth Pa is Hiring an Aging Care Manager 3 (LG) Pike County Area Agency on Aging Near Pike, PA

THE POSITION

DESCRIPTION OF WORK

As an Aging Care Manager 3 you will be tasked with maintaining a caseload of vulnerable clients who are in need of protection or mental health case management services. Work includes documenting services provided and coordinating medical, social, psychological, or functional evaluations. You will be required to maintain an extensive knowledge and understanding of protective services legislation, rules, regulations, and operating procedures involved in the protective services program, as well as community resources to be able to utilize them in assessing client situations. You may be called into court proceedings where you will have the opportunity to work closely with attorneys for guardianship cases and other delicate personal life situations.

Work Schedule and Additional Information:
  • Full-time employment
  • Work hours are 7:30 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.
  • On-call rotation will be required.
  • Salary:In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS
Minimum Experience and Training Requirements:
  • One year as an Aging Care Manager 2 or Aging Program Assessor; or
  • One and one-half years of professional human services, law enforcement, or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
  • An equivalent combination of experience and training which includes six months of professional human services, law enforcement, or nursing experience.

Other Requirements:

  • You must be able to perform essential job functions.
