An Aging Care Manager 2 is expected to appropriately maintain a caseload of vulnerable consumers. Responsibilities involve exercising initiative and independent judgment in discharging assignments, seeking advice and help with more unusual and difficult cases, and collaborating with other community professionals, agencies, and resources. This position obtains information from the consumer, members of the consumers' families, neighbors, and other persons necessary for the identification of social, economic, emotional, health, or physical problems. Effective communication helps to ensure that consumers receive the care and support they need.
