Our LPN II associates:

  • Perform patient observation, data collection related to patient care, administer medication, treatment and procedures according to the physicians orders and resident care plan.
  • They communicate patient findings to their supervisor.
  • Assume responsibility and accountability for facilitating, communicating and collaborating with the healthcare team to identify and meet the physical, emotional and spiritual needs of the patients.
  • Oversee care provided by CNAs, Care Partners, QMAPs and/or other support staff.
  • Promote the optimal health, well-being and safety of the patient through use of the nursing process and in accordance with patient care standards, guidelines and the State Nurse Practice Act.
  • Demonstrate personal accountability for relationship-based care, organizational mission, and core values.

Qualifications:

In addition to bringing your whole self to the workplace each day, qualified candidates will need the following:

  • Two years of previous LPN experience, preferred
  • Current Colorado state LPN license, required
  • High School Diploma or GED, required
  • BLS through the American Heart Association, required
  • State IV certification required within 3 months of hire or transfer, required
  • Knowledge in nursing and person-centered care concepts, required
  • Ability to demonstrate competency in the provision of individual care of patients based on the patients assessed age and developmental needs

The schedule for this position is three 12-hour shifts per week and rotating weekends(7pm-7am)

Physical requirements:

  • Medium Work - exert/lift up to 50 lbs. force occasionally, and/or up to 20 lbs. frequently, and/or up to 10 lbs. constantly.

$21.28 - $35.72 / hour
