Commonpoint Queens is Hiring an Older Adult Program Social Worker Near Flushing, NY

PRIMARY RESPONSIBILITIES:

Help older adults establish connection to virtual activities, tele-health appointments and interpersonal activities, supporting participants in avoiding isolation and to age in place.
Conduct assessments with older adults in the community.
Collect and enter data.
Provide outreach to isolated older adults.
Assist older adults with technology.
Provide individual counseling in older adults’ homes.
Consult with older adults’ families.
Provide psychoeducation and coaching to older adults.
Perform other tasks as needed.

EDUCATION, EXPERIENCE, AND REQUIRED

● LMSW preferred.

● Knowledge of a variety of operating systems i.e., Apple, Android, Microsoft.

● Experience working with older adults strongly preferred.

● Ability to travel throughout Queens county.

● Comfortable working within participants' homes.

● Strong advocacy and creative problem solving skills.
