Job Responsibilities:

• Perform duties as assigned on personal care service plan
• Provide direct and indirect personal care to client's in the home setting
• Establish and maintain effective communication and a professional relationship with clients, family members, and co-workers
• Observe and report any changes in the client's mental, physical or emotional condition or home situation to supervisor

Job Requirements:

• Must have CNA/HHA Certificate
• Must have up-to-date HIV, Alzheimer’s and CPR Training
• Must have valid driver's license with reliable transportation
• Must display high moral standards of honesty and integrity
• Must complete training and orientation program
• Must successfully complete the required background check and drug screenings
