Comfort Keepers #581 is Hiring a Senior Care Manager Near St. Cloud, MN

Comfort Keepers is a locally owned and operated licensed home care franchise since 2004. With over 800 Comfort Keepers franchises, we are growing and are ranked in the Top 10 overall. This is a Non-medicare and non-medicaid company. Comfort Keepers is looking for a Senior Care Manager to improve the quality of services and support for our seniors/clients. We are a home health care team operating under the Minnesota Department of Health Comprehensive Home Care License. We serve primarily seniors. Our territory includes St. Cloud area as well as Brainerd Lakes and Buffalo. The Senior Care Manager will support clients, family, staff and office co-workers to help deliver quality services and support for our seniors. This is a new position joining our supportive team. 

The Senior Care Manager assists clients in determining the type of and duration of services needed to meet the clients’ needs, as well as conducts continuous reassessments of their plan of care. As a Senior Care Manager, you are the first point of contact between clients and the company. You are responsible for developing solid relationships with clients, building trust and maintaining relationships throughout the client’s tenure with Comfort Keepers. The Senior Care Manager provides continuity of care for clients during and after referral, as well as facilitates the provision of in-home care and support services to clients. They assist in the organization of family resources for the best-in-class care of the client. The Senior Care Manager routinely interacts with other office staff members, as well as clients, family members, community providers/referral sources, insurance companies, case managers, and other third-party payors. The Senior Care Manager drives the business and grows revenue by engaging in community events and Client business promotion.

The Senior Care Manager: develops sales and marketing plans, and actively assists in driving revenue growth.

Takes accurate referral/service request information by phone.

Sells prospects on services and schedules care consultations based on the clients’ needs

Contacts clients and caregivers for follow-up service delivery and determines satisfaction of services.

Assesses and acts to resolve client concerns in a manner that embodies the company’s philosophy of “elevating the human spirit”

Documents all required information for regulatory/compliance purposes in the appropriate information systems.

Develops, implements, and evaluates programs and initiatives in order to meet sales, marketing, financial, quality, and service goals.

Processes data necessary to establish initial client records, services, files and initiate invoicing.

Assists in determining type and duration of services necessary to meet client needs. Quotes bill rate based on services provided.

Works with Scheduling to ensure shifts and hours are matched with caregiver qualifications, preferences and availability to client needs.

Visit current clients/client’s homes on a regular basis to ensure quality of care provided meets client’s needs. Participates in client conferences as needed with clinical team.

Analyzes, implements and evaluates current and new services

In conjunction with leadership, ensures appropriate licensure and certification of operations as required.
