Comfort Home Care is the premier psychiatric home health agency in Massachusetts. Our nurses visit a diverse population, mostly with mental health diagnoses, performing nursing assessments, communicating with other providers, and maintaining patients in the community as independently as possible.

LOCATION:

Comfort Home Care is in search of Registered Nurses or Licensed Practical Nurses to work Full Time or Per Diem in Malden and the surrounding areas.

JOB TYPES:

Full Time Shifts are either AM shift (starts at 7am) or PM shift (starts at 3pm).Shifts are Monday through Friday with NO WEEKENDS!!!

Per Diem Shifts are weekday or weekend, on-call or a set schedule, depending on our needs. Design a schedule that works for you.

PAY:

Registered Nurses earn full-time pay of $65,000 - $80,000, with an incentive program that can earn you much more. Benefits and the advantages of a great work-life balance are included.

WORKPLACE:

Comfort Home Care has offices located in Methuen, Everett, New Bedford and Worcester. This is a hybrid role allowing you to visit patients in their homes and then finish your day doing your case management notes from our office or your own home office.

DUTIES AND RESPONSIBILITIES:

Comfort Home Care is seeking Massachusetts licensed Registered Nurses or Licensed Practical Nurse that can:

• RN/LPN provides initial and ongoing assessments to identify the client's medical needs, level of functioning and ability to communicate and learn.
• RN/LPN initiates the plan of care, necessary revisions, and performs OASIS assessments at appropriate time points when applicable.
• RN/LPN Initiates and performs appropriate nursing interventions.
• RN/LPN coordinates services, informs the physician and other personnel of changes in the client's condition and needs.
• RN/LPN collaborates with patient /family and other health care providers and/or community resources with planning of care and discharge.
• RN/LPN Prepares clinical and progress notes and submits all required paperwork in a timely manner according to agency policy and procedure.
