Overview:

We are seeking a dedicated Registered Nurse Case Manager to join our healthcare team. The ideal candidate will provide comprehensive patient care and support while coordinating with healthcare providers to deliver high-quality services.

Duties:

- Conduct patient assessments and develop individualized care plans
- Monitor vital signs and patient progress
- Collaborate with physicians and specialists to ensure optimal patient outcomes
- Provide education and support to patients and their families
- Coordinate referrals to various healthcare services
- Maintain accurate medical records and documentation
- Utilize medical terminology and physiology knowledge in daily practice
- Manage care for patients across different age groups, including pediatrics and behavioral health
- Respond effectively in emergency situations such as the ER

Requirements:

- Valid RN license in the state of practice
- Proven experience in managed care or case management
- Strong background in patient assessment and critical thinking skills
- Familiarity with medical records systems and documentation practices
- Ability to work collaboratively in a multidisciplinary team environment
- Excellent communication and interpersonal skills
- Flexibility to adapt to changing priorities and patient needs

Pay: From $65,000.00 per year

Expected hours: 40 – 60 per week
