Job Overview:
We are seeking a compassionate and skilled Home Care Nurse to join our team. As a Home Care Nurse, you will provide medical care and support to patients in their own homes. Your role will involve Intake Assessments for new clients, assessing patients' health conditions, medication set up, and coordinating with other healthcare professionals to ensure the best possible care.

Duties:
- Conduct physical examinations and assess patients' health conditions
- Administer medications and treatments as prescribed by physicians
- Monitor vital signs and report any significant changes or concerns
- Provide wound care, including dressing changes and wound assessment
- Assist with catheterization and gastric lavage procedures
- Collaborate with physicians and other healthcare professionals to develop and implement individualized care plans
- Educate patients and their families on disease management, medication administration, and self-care techniques
- Coordinate discharge planning for patients transitioning from hospital to home care
- Provide dementia care and support for patients with cognitive impairments
- Perform occupational health assessments for patients returning to work

Skills:
- Strong clinical skills in physical examinations, vital sign monitoring, and medication set up
- Proficiency in catheterization procedures
- Knowledge of discharge planning processes and coordination of care between hospital and home settings
- Experience in providing dementia care and support for patients with cognitive impairments
- Familiarity with occupational health assessments for individuals returning to work after illness or injury

Requirements:
- Valid nursing license in MN
- Minimum 2 years of experience as a Registered Nurse (RN)
- CPR certification
- Excellent communication skills, both verbal and written
- Ability to work independently and make sound decisions in a home care setting

Note: This is a flexible position but will require availability in the AM 2 days a week.

Part time morning in office position available as well (20 hours a week).
