DEFINITION:
The Discharge Planner is responsible for providing discharge planning to patients and the families of patients who are hospitalized or seen in either an inpatient or an outpatient setting. The Discharge Planner coordinates an appropriate discharge plan for inpatients with members of the interdisciplinary team and utilizes community resources. The Discharge Planner may also work with members of the community and make home health visits.
EDUCATION:
Minimum of four years of formal education, Bachelor’s degree in Social Service field with training or experience. Training will include information regarding age appropriate standards for casework with individuals of all ages.
REGULATORY REQUIREMENTS (IF APPLICABLE):
MINIMUM QUALIFICATIONS:
Graduation from Social Sciences Program.
PREFERRED QUALIFICATIONS:
Two years of experience in field of Social Sciences work is preferred.
