The role of the Case Manager is to optimize patient transitions to the most appropriate level of care while facilitating interdisciplinary care across the continuum via assessment of patient/family resources and post-discharge needs. Maintains a highly acceptable level of ethical & professional conduct and respect for medical staff, coworkers, hospital staff, community partners, and third-party vendors. Demonstrates ability to prioritize multiple work assignments in order to accomplish assigned workload. Must be able to work independently, while being part of a highly engaged team.
Experience
Required- 5 year(s): Previous Case Mgmt experience and/or bedside experience in an acute inpatient hospital.
Education
Preferred- Bachelors or better in Social Work
- Bachelors or better in Nursing
- Associates or better in Nursing
Licenses & Certifications
Preferred- Licensed Practical Nurse
- Registered Nurse
