Colorado Kidney Care is Hiring a Nurse Practitioner-Thornton Near Thornton, CO

Job Description:

We are a dynamic medical specialty practice that makes patient care our top priority. Our employees are talented, passionate health care professionals who know how to provide excellent patient care and customer service in our office/outpatient dialysis NP position. We look for people who have a strong work ethic coupled with a positive outlook and a teamwork attitude, with the skills to match. Salary is $99,000-$101,000

Position Responsibilities and Duties:

  • To round, assess and evaluate as directed on each ESRD patient in the dialysis units to which you have been assigned. ESRD patient duties include: chair-side visits and charting of each visit, filling of prescriptions both chair-side and by phone, contacting referring physicians regarding clinical issues, assisting with arrangement of tests when assistance is needed from the dialysis staff to clarify medical issues, and completing annual History and Physicals on ESRD patients as needed.
  • Evaluate plan of care, adjust as indicated.
  • Perform office visits on follow-up patients with CKD.
  • Perform office consults on specified hospital follow-ups who need prompt office follow-up.
  • If the schedule does not conflict with other practice duties, it is expected that the NP/PA will attend monthly CQI and Patient Care Conferences at the dialysis facilities where they are assigned.
  • Patient education both in office and via phone calls to communicate physician instructions regarding; medication changes, requested diagnostic follow-up/labs, diet changes, next visit, etc.
  • Diabetes education including: diet, exercise, monitoring, medication management, problem solving, coping strategies, and co-morbid risk reduction strategies.
  • Entry, Review, Refill and Maintenance of medications in EHR
  • Oversee Infusion Administration Process
  • EHR software patient care data entry must be accurate and timely; you are expected to document all patient care interactions and pertinent medical record information in the EHR charts. This includes both in person office visits as well as phone calls with the patient.
  • Answering and triage of phone calls from patients seeking advice/clarification that the office staff cannot answer.
  • Maintenance and upkeep of incoming messages via EHR and email; this includes timely responding and follow up
  • Cross coverage of other duties as needed, and other duties as assigned
  • Assist with patient referrals and coordination of care regarding dialysis, transplant, access and dietary services.
  • Maintain contact with patients to ensure anticipated therapeutic objectives are being met (i.e. weight, BP, diet, labwork) and communicate with treating nephrologist regarding objectives.
  • Assist physicians with EHR by annotating physicians completely on all interactions and patient care issues regarding their patients.
Experience and Skills:

Essential Skills and Experience:

  • Graduated from an accredited school of nursing, or physician’s assistant program
  • Colorado State RN license in good standing (if an NP)
  • Must have passed the NP or PA certification exam. NPs must be registered with the state of CO Board of Nursing. PAs must be registered with the CO Medical Board.
  • At least 2 years of professional experience with renal patients.
  • Excellent interpersonal, organizational and communication skills required.
    • Ability to foster teamwork and collaboration across departments and within your own department.
    • Strong computer skills and ability to learn new software quickly and easily. Knowledge and experience with EMR systems is preferred.
    • Communicate verbally and in writing in a precise, clear, and professional manner
    • Maintain a high level of professionalism in appearance, behavior, and communication
    • Compassion for patients and the desire to help them
    • Possess and display a high level of patience in all situations
    • Effectively and professionally prioritize and multitask in a fast paced, sometimes stressful, environment
    • Possess an intermediate skill level with Microsoft Word, Excel, and Outlook
    • Navigate the internet proficiently and in a professional manner
    • Have an exemplary work ethic
    • Possess the skills to work with others in a cooperative and supportive manner
    • Ability to function at a high level of efficiency without supervision
    • Maintain a high level of organization and attention to detail
    • Advanced Practitioners are expected to maintain current CPR certification

Work Environment: Work is performed in a fast-paced office environment with patient regular patient contact on the phone and/or in person. The noise level in the work environment is usually moderate
