Job Description:
Colonial Skilled Nursing is currently seeking a dedicated and experienced Social Services/Admissions Director to lead our admissions team and oversee social services functions. The Social Services/Admissions Director plays a pivotal role in ensuring seamless admissions processes and providing comprehensive social services support to our residents and their families.

Responsibilities:

• Lead the admissions process for new residents, including conducting assessments, coordinating with healthcare professionals, and ensuring compliance with regulatory requirements.
• Develop and implement strategies to maximize occupancy and streamline admissions procedures while maintaining high standards of care.
• Provide leadership and supervision to admissions staff, including training, performance management, and professional development.
• Oversee the social services department, including assessment of residents' social and emotional needs, development of care plans, and coordination of support services.
• Collaborate with interdisciplinary team members to address resident concerns, facilitate care transitions, and promote resident well-being.
• Maintain accurate and up-to-date records of admissions, assessments, and social services interventions in accordance with facility policies and regulatory guidelines.
• Serve as a resource for residents and their families, offering support, guidance, and referrals to community resources as needed.
• Participate in facility meetings, committees, and quality improvement initiatives to enhance resident care and satisfaction.
• Stay informed about changes in long-term care regulations and industry trends, ensuring compliance and best practices.

Qualifications:

• Bachelor's or Master's degree in social work, psychology, or related field required.
• Minimum of [insert number] years of experience in admissions, social services, or case management in a skilled nursing facility or similar healthcare setting.
