Under the direction of the Wound Care Center Clinical Nurse Manager, the Registered Nurse/Case Manager provides patient care and handles intake and discharge processes for the Wound Care Center patients as scheduled. The position may also serve as a Case Manager to a group of assigned Wound Care Center patients, as assigned and depending on the Wound Care Center setup.
Essential Functions and Responsibilities:
The RN/Case Manager will be able to demonstrate the following essential competencies, duties, and responsibilities as a result of education, experience and Advanced Wound Clinic initial orientation and ongoing corporate learning.
o Performing the initial nursing assessment, communicating findings with the assigned physicians, assessing the wound stage and measurements, documenting care plans, evaluating the patients’ status in regards to desired outcomes, and identifying patient care situations that require intervention.
o Participating in quality improvement initiatives, providing education to the patient or caregiver, and serving as a patient advocate in the delivery and coordination of the patient care.
Position Requirements:
Education:
Associate Degree in Nursing required; Bachelor's Degree in Nursing preferred.
Licensure/Certification/Registration:
Current licensure in the State of South Carolina as a Registered Nurse or Multistate Compact Registered Nurse License.
American Heart Association BLS or American Red Cross BLS – Healthcare provider required within two weeks of hire.
ACLS certification within 90 days of hire.
Experience:
Minimum of two years current experience in medical-surgical acute care, with preference given for experience in Wound Care.
Wound Care Certification or Wound Care/Home Health experience preferred.
