Job Description:
Collaborative Home Care is looking for an experienced, compassionate, and outgoing caregiver who is currently available for 12-hour Day & Evening Shifts during the week and/or weekends. We are looking to fill immediately to help an elderly woman living in Shelton, CT, who is coming home from rehab. If you are passionate about making a difference in the lives of elderly individuals and meet the requirements listed, we would love to hear from you!
Primary responsibilities:

1. Assistance with transfers
2. Assistance with showering & dressing
3. Light housekeeping and meal preparation
4. Assistance in toileting
5. Client has no pets and does not smoke

Qualifications:

• A love for the senior community
• A compassionate, upbeat attitude and patient demeanor
• Strong commitment to reliability and ethical behavior
• Ability to maintain confidentiality and professionalism
• Ability to communicate effectively in both written and verbal forms
• Dedication to professional development
• A minimum of 1-year experience in caregiving for seniors
• Ability to lift 30-40 pounds
• Possess a valid Driver's License with a clean driving record, Auto Insurance and a reliable and well-maintained car
• Possess a State ID, and a valid second form of ID, such as a Social Security Card, Passport, or Birth Certificate
• Eligible to work in the United States for any employer
• Pass a background check and drug testing per company standards

Responsibilities and Duties:

• Our Caregivers are responsible for providing hands-on, non-medical, quality care to clients of Collaborative Home Care.
• Caregivers provide care based on the plan of care for their clients, which includes activities of daily living such as personal care (e.g. bathing, dressing, medication reminders)
• Companion care (e.g. conversation, hobbies and activities, shopping).
• Caregivers must be able to work with minimal supervision, accepting personal responsibility for the maintenance of a professional relationship with the client in accordance with Collaborative Home Care policies and standards.
