Job Title: CNA, PCA, HHA Caregiver (Dementia Experienced) Company: Collaborative Home Care Location: New Haven, CT Pay Rate: $20 per hour Schedule: Availability from Mondays Through Thursdays from 1:30PM to 4:30PM Job Description: We are currently seeking a caregiver with dementia experience who embodies a go-getter attitude, reliable, someone full of energy, and always up for any challenge for an elderly woman who lives in New Haven, CT. The client needs help with mobility assistance, showering, dressing, meal preparation, and light housekeeping. She has two cats and does not smoke. The client also needs transportation to local errands, so you must be able to drive and have reliable transportation (Your miles driven will be reimbursed). If you are interested, please apply! Responsibilities and Duties: Our Caregivers are responsible for providing hands-on, non-medical, quality care to clients of Collaborative Home Care. Caregivers provide care based on the plan of care for their clients, which includes activities of daily living such as personal care (e.g. bathing, dressing, medication reminders) and companion care (e.g. conversation, hobbies and activities, shopping). Caregivers must be able to work with minimal supervision, accepting personal responsibility for the maintenance of a professional relationship with the client in accordance with Collaborative Home Care policies and standards. Qualifications:

Ability to communicate effectively in both written and verbal forms
Dedication to professional development
A minimum of 1-year experience in caregiving for seniors
Ability to lift 30-40 pounds
Possess a valid driver's license, access to reliable transportation and valid auto insurance
Possess a State ID, and a valid second form of ID, such as a social security card, birth certificate, or passport
Eligible to work in the United States
Pass a background check and drug testing per company standards
