Codman Square Health Center is Hiring a Lead Clinician for Triage and Access Near Dorchester, MA

Reporting directly to and coordinating closely with the Director of Behavioral Health, the lead clinician is a core member of the BH Department with clinical, supervisory, and programmatic responsibilities.

The Lead Clinician for Triage and Access- provides integrated behavioral health care to patients and families of all ages within a primary care setting, offers clinical supervision to BH clinicians, interns, and allied staff, and collaborates with various stake holders to triage patients for acuity, build and maintain community referral partnerships, and monitor clinician schedules and productivity.

The Lead Clinician for Triage and Access works to improve the behavioral health of Codman’s patient population by caring for individual patients, developing BH staff, and taking a holistic approach to BH service access. This Lead Clinician supports the health center in achieving population-based quality of care measures and understands that every health center patient is a potential BH Department patient.

Staff Supervision and Development: Provide clinical supervision for up to 4 supervisees, which may include interns, pre-licensed clinicians, family partners, and case managers. Provide clinical support and feedback to staff to support staff development and growth. Support pre-licensed clinicians in preparing for licensure. Review and monitor supervisees’ clinical documentation including progress notes, diagnostic evaluations, and treatment plans to meet compliance, regulatory, and insurance standards. (25%)

Triage and Access: Collaborate with family partners, case managers, BH clinicians, psychiatry providers, and primary care providers to triage patients for clinical urgency and appropriate treatment setting, including referral out to community partners. Develop, maintain, and solicit feedback on referral relationships to partner agencies including in home therapy, outpatient BH practices, and specialty BH clinics. Support the BH Director in monitoring and maintaining BH clinician and intern productivity. (25%)


Qualifications:

  • LICSW or LMHC licensure with 2 years post-independent license experience (required)\
  • At least two years' experience providing clinical supervision (preferred)
  • Culturally humble and informed clinical therapy experience with patients across the lifespan through therapy services including individual, couples, family, and group work (required)
  • High ethical standards and the ability to maintain professional licensing (required)
  • Able to successfully complete insurance credentialing (required)
  • Strong clinical documentation skills incorporating DSM-5-TR diagnoses and facilitating communication with patients and medical providers (required)
  • Strong time management, systems thinking, and communication skills (required)
  • Experience with integrated behavioral health in primary care (required)
  • Experience with electronic health records (required) and Epic (preferred)
  • Fluency in Haitian Creole or Spanish (preferred)
