Expectations of all Employees

  • Demonstrate an understanding of and adherence to appropriate and ethical behavior as stipulated in the CODAC Policy and Procedures Manual
  • Maintain a professional, positive, and respectful attitude with all work-related contacts
  • Communicate regularly with the supervisor
  • Meet productivity standards and perform duties as workload necessitates
  • Always maintain client confidentiality and ensure to follow agency, state, and HIPPA laws
  • Demonstrate an understanding of and adherence to confidentiality regulations as stipulated in the CODAC Policy and Procedures Manual, Federal Regulation 42CFR part 2, and other pertinent Federal and State laws and regulations
  • Ability to be flexible and adaptable to change.

MAJOR DUTIES AND RESPONSIBILITIES:

Counselor:

  • Maintain individual caseloads following CODAC policy and procedures
  • Provide counseling, case management, risk assessment, crisis intervention, patient education, facilitating referrals, discharge planning, and follow-up aftercare.
  • Maintain patient EMR and all required clinical documentation, including but not limited to Clinical progress notes, screenings, intake, assessments, treatment planning,
  • Actively monitor client risk factors and take action to ensure client’s safety while following Codac policies and procedures
  • Provide case presentations at clinical meetings
  • Implement standardized evaluations or assessments in conjunction with multidisciplinary teams as appropriate
  • Ensures required all clinical documentation is complete, timely, and accurate at all times
  • Obtain and track the proper Level of Care and Acuity as needed to provide services
  • Maintain caseload productivity goals for Direct Services

IOP Facilitator:

  • Monitor urine and breathalyzer tests when necessary including coordinating random urine screens
  • Collect and organize monthly data on enrollment and attendance
  • Update attendance rosters as enrollment changes
  • Update and coordinate care with internal and external referral sources
  • Scheduling intakes
  • Provide progress updates
  • Coordinate and facilitate monthly IOP clinical meetings, or as needed
  • Assist in onboarding and training of new IOP facilitators
  • Being a point of contact for IOP facilitators to assist with scheduling, group coverage, technical issues or support, and any other questions or problems including during IOP group hours
  • Continue building program on an ongoing basis
  • Outreach to create and maintain relationships with IOP and group referral sources, including speaking engagements when required

GENERAL

  • Complete Prior Authorizations as needed
  • Establish and maintain good working relationships with community agencies.
  • Attends and participates in team conferences, as needed
  • Serves as a behavioral health resource to interns (if applicable)
  • Attends supervision meetings as required
  • Monitor urine and breathalyzer tests when necessary
  • Obtain and/or maintain appropriate credentialing and licensure
  • Provide basic office tasks, if necessary
  • Obtain and maintain Motivation Interviewing proficiency
  • Participate in Performance Improvement activities
  • Perform other duties as required

QUALIFICATIONS:

Masters Degree in a Behavioral Health Care Field. Minimum of 2 years experience in Behavioral Healthcare
