Job Overview:
We are seeking a Licensed Practical Nurse to join our healthcare team. As an LPN, you will play a vital role in providing quality care to patients. Your responsibilities will include assisting with patient care, administering vaccinations, and collaborating with the healthcare team to ensure the best outcomes for our patients.

• Handles telephone inquiries from patients and/or caregivers regarding test results, medications and treatment instructions as directed by the provider.
• Arranges for hospitalization, referrals, or performance of special procedures for patients obtaining prior authorizations when needed.
• Maintains a clean, attractive work area and keeps clinical areas stocked with appropriate medical supplies.
• Considers age-related factors in the care, treatment, assessment and education of patients.
• Learns and maintains up to date knowledge of the Electronic Health records system and scheduling system used by Coalfield, is able to operate efficiently and accurately as needed.
• Participate in care coordination as needed, research as needed, educational or outreach activities upon request
• Assists physician or nurse practitioner with prioritizing lab work review, calling prescriptions to pharmacy, and necessary paperwork completion.
• Maintains policy and procedures, safety, and environmental infection control standards conforming to OSHA regulations.
• Monitors and ensures accuracy of documentation on patient medical records on a timely basis.
• Ensures and seeks to improve nursing standards and nursing care practices to meet new office practice concepts and managerial methods.
• Assist Chronic Care Management Nurse and Quality Nurse to improve patient health.
• May be asked to greet patients and prepares them for physicians’ or nurse practitioner’s examination. Screens patients for appropriate information. Instructs patients in collection of samples and tests.
• May be asked to administers prescribed medications and treatments in accordance within the scope of the licensed practical nursing standards.
• Completes prior authorizations as required for medical supplies, medications, etc.
• Serve as patient advocate to improve quality of care
• Follow all Coalfield Health Center policies and procedures.
• Performs other duties as assigned or requested.

Qualifications:

• Valid LPN license in West Virginia

Minimum of 2 years’ experience in pediatrics required.

• Vaccine for Children program experience preferred.
• Experience with prior authorizations preferred.
• Experience completing referrals required.
• Minimum of 5 years’ experience working as an LPN preferred.
• Must be proficient and able to use computers, including electronic health records.
• Must have the ability to work in a team setting.
• Registered Nurses may be considered.

Job Type: Full-time

Pay: $15.00 - $20.00 per hour
