Description

POSITION SUMMARY:

The responsibility of the Charge Nurse is to provide direct nursing care to the residents, and to supervise the day-to-day nursing activities performed by nursing assistants and certified medication aides. Supervision must be in accordance with current Federal, State, and Local standards, guidelines, and regulations that govern the facility. To insure quality patient care consistent with the philosophy and objectives of the organization. This position provides direct operational support for the department on a shift basis, acts as a clinical resource person for the staff and acts as a liaison between staff and management, assisting the DON and/or ADON in determining staffing needs and identifying problems within the nursing department. The position serves as a resource to staff for patient care assessment and in the planning of nursing interventions. The position facilitates problem and resolution and collaborates with the DON and/or ADON in assessing quality of care provided, identifying staff strengths and weaknesses, and in evaluation of staff performance. Operational issues are identified by this position and the individual works to take action and correct problems. Interface with physicians and interdisciplinary teams for comprehensive care planning is also required.

ESSENTIAL FUNCTIONS:

  1. Demonstrates knowledge of CCGH/CCEC standards of practice, policies and procedures, and other department specific guidelines.
  2. Provides on-going assessment of the residents physical, psychological, and social needs to determine levels and types of care.
  3. Demonstrates the ability to deliver care based on identified needs and physician orders.
  4. Prioritizes and delegates department activities as well as patient care activities to deliver safe, effective care in a timely manner.
  5. Administers activity within the department in a fair and efficient manner.
  6. Cognizant of and adapts to the changing trends in patient care on a shift basis.
  7. Supervise all staff, orienting to the area, assessing competencies, and skill knowledge.
  8. Assists other team members in providing patient care when professional skill and judgment is required.
  9. Monitor staff behavior in public areas and intervening if indicated, so the patient, physician, family or other customers only see on stage behavior.
  10. Interface with upset customers to problem-solve for good resolution of issues.
  11. Coordinates resources for staff with clinical questions, to assure the patient care is being completed, call light answered, medications given, physician questions answered, and patient care coordinated.
  12. Mentor new staff and seasoned staff on specific clinical issues, physician concerns and different equipment or policies.
  13. Periodically review the residents written discharge plan. Participate in the updating of the plan as necessary.
  14. Communicate department needs effectively to other leadership staff members.
  15. Assist DON and/or ADON with evaluation of staff. Provides counseling as necessary and in private.
  16. Participate in survey process made by authorized government agencies.
  17. Demonstrates the ability to provide written descriptive nurses notes that reflects the care provided to the resident, as well as the residents response to the care.
  18. Follows established procedures for charting all accidents/incidents involving residents, staff or visitors.
  19. Completes and files required record keeping forms/charts upon the residents admission, transfer, and/or discharge.
  20. Receive telephone orders from physicians and record on the Physicians Order Form.
  21. Demonstrates the ability to record new/changed diet orders. Forwards information to the dietary department.
  22. Transcribe physicians orders to resident charts, cardex, medication cards, treatment care plans as required.
  23. Report all discrepancies noted concerning physicians orders, diet change, charting error, etc. to the ADON.
  24. Prepare and administer injectable medications as ordered by the physician.
  25. Demonstrates the ability to verify the identity of the resident before administering medication and/or treatment.
  26. Orders prescribed medications, supplies, and equipment as necessary, and in accordance with established procedures.
  27. Ensures narcotic records are accurate for the shift.
  28. Dispose of drugs and narcotics as required, and in accordance with established procedures.
  29. Notifies the attending physician of automatic stop orders prior to the last dosage being administered.
  30. Reviews medication cards for completeness of information, accuracy in the transcription of the physicians order.
  31. Reports any changes in patient condition to the physician.
  32. Performs any other duty as assigned.
