PERFORMANCE RESPONSIBILITIES:
(other duties may be assigned as determined by the Superintendent)

1. Attend student registration to provide information on medication and clinic policy.

2. Attend beginning of the year open houses in each school and have clinics open for tours.

3. Develops individualized health plan with specific goals and interventions unique to the diagnosis and the student, with input from family and physician.

4. Administers prescribed medication according to Indiana Code (IC 34-4, 16.5-3.5)

5. Coordinates training of paraprofessionals—extent of orientation and amount and type of supervision needed to comply with State Nurse Practice Act and other legal considerations (IDOE and ISDH guidelines for delineation of duties for Children with Special Health Care Needs).

6. Follows the guidance provided in the Indiana School Nurse Manual when delegating nursing act responsibilities and provide necessary training for paraprofessionals to ensure student safety.

7. Prepares and submits reports for the Superintendent and the Indiana Department of Education.

8. Assists in development and revision of the health curriculum.

9. Administers first aid and emergency care as defined in the Indiana State Department of Health’s First Aid Guidelines for Emergency Care of Illness and Accidents Occurring at School manual.

10. Upon request provides formal health instruction within the classroom, based on sound learning theory, as appropriate to student development level in subject areas such as: Growth and Development; AIDS and other Sexually Transmitted Diseases; Cardio-Pulmonary Resuscitation; hygiene; first aid; and other health concerns.

11. Delivers, organizes, and maintains the first aid supplies and AED supplies in the schools.

12. Assists in the social service referral process for individual students and/or their families.

13. Performs clerical tasks such as filing, ordering supplies, data entry, and telephone communication with parents, staff members, and community agencies.

14. Maintains immunization and health records of students.

15. Evaluates the health records of all new students to ensure that all immunization requirements are met. Ensure that all required information is obtained.

16. Maintains all required state reports.

17. Participates in case conferences as needed.

18. Performs scoliosis tests on students as required by state law and also on any other student as needed.

19. Treats students and staff visiting the clinics on a daily basis.

20. Supervises the maintenance of daily visit logs, daily medication logs, and other information as necessary.

21. Maintains bee-sting plans on all students with known allergies to stings.

22. Coordinates Hepatitis B and Influenza Vaccines for staff.

23. On-call to respond to emergencies at all schools.

24. Communicates all health issues to the appropriate teachers.

25. Updates policies and procedures regarding clinic protocols as needed.

26. Supervises vision and hearing screenings for students and sends individual referrals to parent/guardian.

27. Assists with school dental screenings and referrals.

28. Checks for head lice as needed.

29. Sends updates to the office secretary, transportation coordinator and school administrator stating who is to be excluded due to head lice and their bus number.

30. Follow-up, educate, and re-check for head lice as needed.

31. Authorizes exclusion and readmission of students in connection with infectious and contagious diseases.

32. Maintain confidentiality.

33. Maintains a regular and reliable level of attendance in order to perform and complete duties as outlined above.

34. Coordinates drug testing of all 9th, 10th, 11th, and 12th grade students, including obtaining and maintaining consent for drug testing, randomly selecting students for monthly testing, testing students, and advising the Drug Education Coordinator of positive results, and retesting as needed per the Cloverdale Student Drug Policy.

35. Coordinates drug testing of all DOT drivers.

36. Will provide all necessary training for Health Aides during the last full week of July each year.
