The Home Health Aide, under the supervision of the Director of Nursing, performs patient care as directed by way of a written plan of care and in accordance with state practice guidelines.

Looking for an experienced Home Health Aide to provide assistance in daily living & personal care.

Locations: West side, South side, North & East side of Indianapolis

Home Health Aide Responsibilities:

●Assist clients with activities of daily living (ADL's) and follows clients' plan of care accurately.

●The Home Health Aide complete's all documentation appropriately and on time in accordance with Clovers policy and procedures.

●Reports any changes in clients' health both physical or mental to the DON/ADON.

●The Home Health Aide provides personal care including but not limited to bathing, shampooing hair, shaving, oral care, dressing, changing linens, peri care, light house keeping, meal preparation, taking vitals, & med reminders.

●Transfer clients in and out of bed, bath, wheelchair and/or vehicle. Utilizes equipment safely and effectively to meet client needs.

●The Home Health Aide engages the client with meaningful conversations, provides emotional support and companionship.

●Follows company guidelines: Dress Code, Attendance, HIPPA Compliance, Communication with Clients, Family Members and Clover Staff.

●Responds to emergency situations.

Requirements for Home Health Aide:

• Valid driver’s license, car insurance, & reliable transportation with valid registration.
• Positive, motivated, and eager to learn!
• CPR certified (Don’t worry if you’re not certified now! We have resources to help).
• Proof of a TB test within the last twelve months.
• 1-2 years of experience as a caregiver.
