Pay Package

• Local Weekly Rate-$1987, Local Hourly Rate-$49.68

Shift&Schedule

• 5x 8 hour shift
• 7am to 3pm, 3pm-11pm, 11pm-7am (All shifts available)

Duties

• Provide skilled nursing care to patients requiring professional nursing services
• Assess, plans and implements client care by following Plan of Care or Doctor’s orders. Doctor’s orders may consist of Dressing changes, Insertion and irrigation of urinary catheters,Oxygen administration by mask or cannula, Tracheostomy care and endotracheal suctioning,Gastrostomy tube feedingsGastrostomy/Jejunum tube medication and flushes, SQ or IM injectionsInsulin administrationObtain SpecimensNephrostomy tube flushesColostomy care and/or irrigationAdministers, CPR and other emergency procedures as necessary and provide staff training and in-service
• RN’s may have to take the role of staff development nurse and nurses who perform episodic nurse duties in times of need
• RN’s may need to step in and perform LPN duties when needed.
• RN’s supervise all LPN’s clinically.
• Completes assignments in an efficient manner and consistent with the time frames allotted.
• Provides back up nursing coverage when primary nurse is off duty and contacts the home(s) to determine health care needs at least weekly.
• Maintains a current Nursing Policy and Procedure Manual; ensures its accessibility to staff within assigned homes.
• Maintains certification in Infection Control for Health Care Workers.
• RNs’ supervise the following LPN Duties.
• Consistently attends to health needs of all individuals.
• Institutes physician’s orders as appropriate and advises Residential Nurse of new orders for medications and/or treatment
• Prepares, administers and documents all medications and treatments prescribed by physician’s orders, promoting independence based on self-medication administration assessments and habilitation plans.
• Administers insulin and monitors blood glucose levels via glucometer, administers injectables and medications via tube (G/J/PEG) as well as administering and ensuring accountability of controlled medications. Documents as required.
• Follows standard procedure for ordering and receiving medications. Documents using proper forms.
• Implements catheterization orders, tracheostomy care, pharyngeal suctioning as well as any additional medical nursing treatment as deemed necessary and within LPN standards of practice.
• Documents health concerns (including seizures, menses, vital signs including temperatures, BMs, unusual behavior, etc.) which may indicate health care problems; gathers facts and reports significant information to the appropriate health care providers. Attends medical appointments as assigned.
• Assists staff with concerns or questions regarding individuals’ care within the LPN scope of practice

Requirements

• Valid RN license is required
• At least 2 years of recent experience as a Registered Nurse is required
• 2 years of Case Managemnt experience required
• Current resume is required
• Current BLS required
• 2 current clinical references are required

Job Type: Contract
