Clinical & Support Options is Hiring a Shelter Community Support Provider, Springfield Near Springfield, MA

CSO is looking for a Shelter Community Support Provider to work with shelter guests in Springfield. 

The Shelter Community Support Provider will provide intensive, clinical casemanagement to consumers with serious mental illness needing such support, orwho have been hard to engage in the mental health system. Services will be provided at shelter or in anoutreach setting. The Shelter CommunitySupport Provider will have regular contact with consumers, and will providesupport services for family members and others in the "natural helpingnetwork."

Responsibilities

  • Provideskills training and assistance in areas of daily living activities includingtransportation, money management, cleaning, laundry, food preparation, personalgrooming, and health care.
  • Assistwith advocacy needs; facilitate "mediation" as appropriate.
  • Provideassistance in identifying and obtaining entitlements, which meet consumer'sneeds.
  • Providecounseling and intensive support to consumers during psychiatric crises orperiods of transition.
  • Assistconsumer with identifying appropriate mental health and medical services and accessingservices of choice.
  • Provideassistance with developing and maintaining a "natural supportnetwork" of family and friends.
  • Assistconsumer in maintaining the appropriate upkeep of apartment or shelter space thatwill ensure healthful and safe living.
  • Assistconsumer in negotiating relationships with others.
  • Coordinatedevelopment of meaningful day activities, including employment or volunteeractivities.
  • Assist with finding or developingrecreational and social activities of the consumer's choice.
  • Provide transportation for consumers asappropriate, following program guidelines.
  • Maintain appropriate contacts withfamily members, others in the "natural helping network", outpatienttherapists, and others as necessary to support consumers.
  • Participate in team discussionsregarding program consumers in order to provide back up for other team membersas needed.
  • Participate in design, review, andrevision of treatment plans with consumer, family members, and other providers.
  • Participatein team meetings, inter-program and inter-agency case conferences, and otherinteractions with other providers to promote integrated consumer services. Receivesupervision on a regular basis.
  • Maintain neat, accurate, and timelyrecords in accordance with agency policies and procedures along with allregulatory bodies. 
Qualifications

Bachelor's degree in a mental health discipline plus twoyears' experience in community mental health setting preferred. Associate's degree and two years' experienceor high school diploma and five years' related experience required. Experience treating individuals with aserious mental health illness strongly preferred. Must have excellent oral and writtencommunication skills, and computer competency. Must be able to work independently and follow through with projects. Must possess valid driver's license andreliable transportation.
