Clinical & Support Options is Hiring a Scattered Site Housing Case Manager, Springfield Near Springfield, MA

CSO is looking for a Scattered Site Housing Case Manager to join our team at Friends of the Homeless in Springfield. 

The Scattered Site HousingCase Manager (SS Case Manager) provides outreach and coordinates/monitors theassessment, treatment planning process and follow-up for each tenant of thescattered site permanent supportive housing units located throughout theHampden County region. The position serves as advocate and liaison for tenantbenefits, and is also part of the Housing Department's service team, workingcollaboratively with the Coordinated Entry System (CES) and assisting with housingplacement. 

Responsibilities

  • Assistwith the program development of this housing project by identifying units forrent and work closely with the Coordinated Entry System (CES) that willidentify homeless individuals prioritized for these units, and facilitate theoccupancy of participants that move into the units.
  • Accessthe CES data system to receive referrals for prospective tenants as units areavailable, and regularly attend and participate in CES case conference meetingsto represent FOH as a PSH provider of units in the Continuum of Care (CoC).
  • Completenew tenant assessment to determine eligibility and needs for social supportprograms.
  • Assisttenants in the development of individual plans to move toward self-sufficiencyand to retain/secure temporary/permanent housing.
  • Evaluate,facilitate and monitor tenant's progress towards established goals andobjectives. Maintain and update tenant information in the agency's data system.
  • Provideresource/assistance for tenants in budgeting, job training, employment search,locating permanent housing, managing medical issues and addressingtransportation and other needs.
  • Advocatefor tenants with other serviceproviders, including federal, state and city agencies and other non-profits;provide referrals to other appropriate service providers as needed.
  • Maintaincurrent knowledge of and good working relationships with other serviceproviders with whom FOH and CSO collaborates.
  • Maintainconfidentiality and appropriate professional boundaries.
  • Ability to multi-task,prioritize, organize and manage time effectively.
  • Provide follow up andcrisis intervention for assigned tenants.
Qualifications

Bachelor's degree in social work or relatedfield or relevant experience required. Two years' work experience in housing search and/or case management. Demonstrated ability to work independently andas part of a team. Strong written andverbal communication skills. Problemsolving and conflict resolution skills. Computerskills with proficiency in Microsoft office programs. Ability to work with diverse population andexercise good judgement. Maintain valid driver's license and an acceptable driving record.
