CSO is looking for a Case Manager to join our Family Support Services team in Springfield.

Looking for a rewarding opportunity to better the lives of children and families in your community? Join our team of dedicated professionals in our Family Support Services. Family Support Workers provide community-based direct care services to children, adults, and families. This includes crisis stabilization, ongoing support and case management through the state's Children's Behavioral Health Initiative. The main focus of these services is to alleviate the youth's mental health issues and strengthen the family structures and supports. Family support services are distinguished from traditional therapy in that services are delivered in the home or community and the frequency and duration matches the need and is not time-limited.

CSO provides a Professional and supportive work environment, unrivaled onsite training, meaningful and supportive supervision, flexible schedules and a strong team environment.

Responsibilities

  • Provide skills training and assistance in areas of daily living activities including basic living, interactive and parent skills. Assist in developing communication and conflict resolution skills and act as a role model for interpersonal relationships.
  • Help parents meet children's daily needs; observe and document child growth and development.
  • Assist with family advocacy needs; facilitate "mediation" as appropriate.
  • Provide counseling and intensive support to consumers during psychiatric crises or periods of transition.
  • Assist consumer with identifying appropriate mental health and medical services and accessing services of choice.
  • Provide assistance with developing and maintaining a "natural support network" of family and friends.
  • May provide Behavior Monitoring services to youth, including implementation and monitoring of the youth's behavior treatment plan, ensuring reinforcement of the plan by the parent/guardian/caregiver, reporting progress to, and working closely with the Behavior Management Clinician to ensure behavior management plans are implemented as developed.
  • May provide Therapeutic Mentoring services to youth, including role playing, teaching of alternative strategies, provision of anticipatory guidance, behavioral rehearsals, and exposure to social situations in which age-appropriate skills can be practiced.
  • May provide Recovery Support Navigation services including exploring treatment recovery options with clients, developing a set of goals and objectives in conjunction with the client based on identified needs, providing education and resources, and assisting clients in accessing treatment and community supports.
  • Provide transportation for consumers as appropriate, following program guidelines.
  • Maintain appropriate contacts with family members, others in the "natural helping network," outpatient therapists, and others as necessary to support consumers.

Qualifications

Bachelor's Degree in a mental health field, experience working with children and families, computer proficiency to use electronic health record and other information systems, valid driver's license and reliable transportation.
