The Nurse Director Case Management will work for a nonprofit hospital system that provides hospital based care, emergency care, primary care, and specialty care at 20 locations throughout the region. The RN Director Case Management will oversee 20 FTEs and will have the support of a Hospital Lead Case Manager and experienced RN Case Management staff.. The position will report to the Senior VP of Clinical Operations. The Case Management RN Director will provide administration leadership over Utilization Review, Integrated Clinical Service, Discharge Planning, and Care Transitions. This position will maximize access to and efficient utilization of resources with a focus on correct patient placement status, patient throughput, length of stay, and safe, timely transitions of care.
The Case Management Nurse Director will have to take an active leadership role in daily interdisciplinary patient rounds and LOS meetings to facilitate timely, appropriate, cost-effective patient transitions with in-depth knowledge about Medicare, Medicaid, and commercial claims. The ideal candidate will be someone who takes the initiative to lead and communicate with staff in a compassionate yet focused manner. The department has a culture of taking ownership and accountability for the work being performed.
