The Regional Heart, Vascular and Thoracic Institute- Hillcrest Hospital has a full-time opportunity for an Experienced Advanced Practice Provider!
This Provider will be dedicated to the coordination of improving patient outcomes by providing an expert-led, patient-centered framework of care designed to provide an accurate diagnosis with active surveillance of valve pathology and timely referral for intervention. Integral part of a team of interventional cardiologists, cardiac surgeons and imaging specialists to coordinate the screening, testing and scheduling of the TAVR evaluation and procedures. Incudes management of TAVR consults, testing results and pre/post procedure schedule management. Outpatient role of frailty/educational visits, pre op visits and post op care visits. Inpatient role of in-patient consults, post care, rounding and safe discharge of the patients. Adherence to TVT registry and best practice guidelines.
Anticipated Schedule: Days, Monday- Friday 8am-4pm. No weekends and No Holidays . (schedule and hours are variable and subject to change based on organizational need)

APRN: Nationally certified licensed healthcare provider who provides primary and/or specialty nursing and/or medical care in a variety of healthcare settings. Functions in collaboration with a physician according to a standard care arrangement and specialty certification. Practice emphasizes health promotion, disease prevention and the diagnosis and treatment of acute and chronic health problems.
PA: Nationally certified licensed healthcare provider who provides primary and or specialty medical care (the practice of medicine) in a variety of healthcare settings. Functions the collaboration agreement of a Physician, (as per O.R.C. 4730) performs complete histories and physicals on patients of all ages to establish and record patient's current health status and to develop a working diagnosis and treatment plan to provide continuing medical care, including daily examinations, writing orders, chart notes, discharging patients and completing discharge summaries. This medical care may be in a variety of clinical settings. PA's are privileged by the Board of Governors. PA's in the Hospital care role provide outpatient and/or hospital based care to patients (more than 50% of their time) with complex, acute, and chronic health conditions. Outpatient/Hospital care PA's practice in a setting that requires the performance of diagnostic and therapeutic interventions to manage acute and chronic health problems. Must have the ability to work with multi-disciplinary teams.
