Collierville Nursing and Rehabilitation is seeking a Social Worker!
While we prefer a candidate to have experience in long term care, a passion for serving the elderly matters most.
This position has oversight of the memory support unit, assists with assessment and documentation of care planning, behavior management and discharge planning, works with residents/groups in promoting quality of life and much more.
