Program: Commons
Location: Rolling Meadows, IL
Hours: Either Full Time or Sub-status
Pay: $33/hour, PAID Training
Sign-on Bonus: $250 bonus on first paycheck, $1,000 at 6 months and $1,000 at 12 months
PM Differential - $2.00 per hour (3:30pm – 12:00am)
Night Differential - $3.00 per hour (12:00am – 8:00am)
Weekend Differential (non-Baylor) - $6.00 per hour (Saturday 8:30am – Monday 8:30am)
The Licensed Practical Nurse (LPN) has the responsibility to render nursing services to clients under the direction of the Director of Nursing and/or Physician. The LPN provides the clients under her/his care with nursing services in accordance with their needs. The LPN supervises the Habilitation Aides in the absence of other supervisory personnel.
Essential Functions:
1. Administration of medications and/or treatments as ordered.
2. Provide training in habits in personal hygiene skills as seen necessary or as a follow up to written care plan.
3. Participate as appropriate in the planning and implementation of client programming.
4. Detect signs of illness, dysfunction, or maladaptive behavior that warrant medical, nursing, or psychosocial intervention. Coordinate with client's QIDP/Program Manager and Coordinators to ensure follow up.
5. Possess basic skills required to meet the health needs and problems of the clients.
6. Provide first aid in the presence of accident or illness.
7. Assist in completing monthly assignments which include:
- Vital signs and weights
- Nursing monthly summaries
- Annual summaries
8. Document services provided to the clients (e.g. bowel, bladder, etc) and changes in the resident's condition.
9. Follow behavior programs and document accordingly.
10. Transcribe physician's orders and type dictation from physicians as necessary.
11. It is the responsibility of the LPN to maintain confidentiality with all clients and their families and maintain a professional image/role as a representative of the
agency.
12. Adhere to all policies and procedures established for Clearbrook, the Commons, and those of all governing bodies.
13. Assist with filing on the day time and midnight shifts.
14. Document significant medical issues about the residents on the com. log.
15. Complete mandatory training annually.
16. Perform any other duties as assigned.
