NURSE PRACTITIONER

CleanSlate is a nationwide medical group providing office-based outpatient treatment for the America’s crisis of addiction. Our Mission is to improve the quality of life for people suffering from opioid and alcohol addictions. We're accomplishing this by providing high-quality medication-assisted treatment (MAT) led by physicians and licensed medical providers, individualized mental health support, psychosocial support, and intensive care coordination with a wide network of community partners. We are currently looking for a Nurse Practitioner for our outpatient substance abuse treatment center located in Kokomo, IN! This position entails the initiation and maintenance of medication assisted treatment primarily, but not exclusively for opioid and alcohol dependence.
