The Medical Navigator will provide assistance to CCA clients and residents of Clatsop County by helping them navigate the healthcare system, identifying available medical resources and assisting with access to medical care. The purpose of this position is to improve health outcomes for CCA clients and reduce barriers to accessing medical care.
Reports to: Social Services Program Manager
Responsibilities
The Medical Navigator will be responsible for:
· Identifying and overcoming barriers to accessing available medical care and resources
· Assisting clients in accessing resources to meet their medical needs, such as prescription discount programs, free/low-cost clinics, medical supplies, and community health education/classes.
· Assisting clients in scheduling appointments, making assessments, and taking next steps
· Helping clients access transportation to and from medical appointments, and accompanying them if necessary
· Assisting clients to plan and prepare for initial and ongoing medical appointments, including providing reminders, arranging childcare, and discussing medical concerns with doctors
· Supporting clients in maintaining their medical treatment plan
· Building knowledge of local medical providers accepting new OHP patients and assisting clients in locating approved providers and navigating insurance and provider requirements
· Assisting clients in reaching out to OHP/CCO for correct enrollment and associated concerns
· Identifying clients who may be eligible for other services – make referrals as required
· Completing mandatory trainings
Qualifications
· Working knowledge of social services
· Ability to communicate professionally through email, telephone, and in-person to clients, throughout CCA, with community agencies throughout Clatsop County and beyond
· Proficient use of computer technology, including windows-based programs, database systems, and internet applications. Commitment to data quality improvement as an ongoing activity.
· Demonstrated ability to work autonomously and as part of team through collaboration and engagement
· Valid driver’s license, proof of insurance, or ready access to reliable transportation, and insurance for business travel
Working Conditions and Physical Effort
· The employee must be able to work well in an office environment. Due to the nature of the work, the employee may be required to deal with angry, confused, and/or frustrated clients.
· The employee must frequently stand, sit, lift, and/or move up to 10 pounds, and occasionally lift and/or move up to 35 pounds.
· Requires considerable walking (including climbing stairs).
