Clatsop Behavioral Healthcare is Hiring a Development Disabilities Service Coordinator Near Warrenton, OR

This position qualifies for a hire on bonus of $1000 with 1/2 paid with 1st check after hire and 1/2 paid with 1st check following 6-month probationary period.

Clatsop Behavioral Healthcare is moving this position to a 4 day 9 hour work week with paid lunches to promote a better work life balance.

Clatsop Behavioral Healthcare has an immediate opening for a Full Time Developmental Disability Services Coordinator. This position advocates for individuals in Developmental Disability (DD) services and makes sure that they are receiving the most suitable services available.

The DD Services Coordinator works with a diverse population and with very specific support needs and identify individual’s strengths and challenges to provide support in a safe manner. Conduct quarterly monitoring of foster homes and group homes of all support being provided within the home.

Essential Duties/Major Responsibilities:

  • Develop Individual Support Plans (ISPs) and Service Agreements that address client goals and level of care needs identified in either an Adult Needs Assessment (ANA), Children's Needs Assessment (CNA), Oregon Needs Assessment (ONA) or Support Needs Assessment Profile (SNAP).
  • Monitor services, client records, and documentation in group homes, foster homes, and/or supported individuals’ own homes to verify standards are being met
  • Develop individual Plan of Care (POC) based on the person's needs assessment and goals identified through the Individual Support Plan (ISP).
  • Benefits counseling and Medicaid enrollment for new and existing clients. Follow up with client/family to discuss approval/declination and services are then discussed and developed based on the results.
  • Identify housing for individuals seeking residential services outside of their family home. This may include children and adult foster homes, group homes, out-of-county placement and supported living. Services are then developed on client desires, needs and interests.
  • Complete Notice of Planned Action and assist individual or family to file hearing request if desired.
  • Advocate for clients’ rights and services.
  • Collaborate with community providers.
  • All CBH Staff are Mandatory Abuse Reporters per the State of Oregon and follow Mandatory Abuse Reporting policies.

Requirements:

  • Bachelor’s degree in behavioral science or related field; or Bachelor’s degree in any field with at least one year of social services related experience; or an associate’s degree in a behavioral science AND two years human services related experience; or three years of human services related experience.
  • Excellent communication skills with individuals and families, community partners and providers and provide excellent customer service.
  • Skilled at working as a member of an interdisciplinary team and completing required documentation.
  • Excellent oral and written communication skills.
  • Strong attention to detail, highly organized and ability to multitask.
  • Valid driver’s license or ability to obtain Oregon or Washington DL

Preferred:

  • Bilingual
  • Knowledge of Health Insurance Portability and Accountability Act (HIPAA) and importance of maintaining confidentiality.
  • Skilled at remaining calm in stressful situation and de-escalating potential aggression from clients.

12 paid holidays and 6.77 hours accumulated per period of PTO years 1-3, increases after year 3. PTO accumulation starts at hire and is available for use as accumulated. Employees are eligible for elective training reimbursement of ($1000/yr for unlicensed clinicians and $1500/yr for licensed clinicians.) CBH also offers sabbatical of 176 hours after 10 years of service and annual elective training fund that includes 40 paid hours for training. Relocation assistance, supervision towards licensure available.
