About Classic Home Care:

Classic Home Care is a nurse owned skilled home health agency based out of Farmington Hills, Michigan that has been around since 2003. Classic Home Care is a Joint Commission accredited, Medicare certified home health agency. Both owners are registered nurses (RNs) and work alongside the team.

We are looking for a registered nurse (RN) for our Full Time Home Health Assistant Clinical Manager position who is a quick learner and a team player. This position is a hybrid/flex position with time shared between the office and seeing patients out in the field.

Full-Time Home Health Assistant Clinical Manager Compensation:

The full time home health assistant clinical manager position is paid at an annual salary of $95,000 plus a $5,000 sign on/retention bonus to be paid throughout the first year.

Weekend/day and Holiday On-Call Visits ($60.00 per point)

Regular Visit/Discharge with Visit/Transfer with Visit/Evaluation Visit= 1 point

Recertification OASIS/Resumption of Care OASIS/Other Follow Up OASIS= 1.25 points

Start of Care OASIS= 1.50 points

Lab Drop Off= 0.25 points (This is not per lab visit that you have, but rather when you drop off lab(s). In other words, if you have two visits today that require a lab, but only go to drop off the labs once, then this is ONE lab drop off).

Weekend On Call: $200 per regular two day weekend / $100 for Saturday and $100 for Sunday.

Home Health Assistant Clinical Manager Job Description:

• Provide care to patients in the community to help them get back to their life of independence.
• Conduct and complete initial assessment of our patients in order to generate a personalized plan of care in collaboration with physicians and the interdisciplinary team.
• Observes signs and symptoms and reports to the physician and/or other appropriate health professionals as often as needed, or upon changes in the patient’s condition.
• Teaches, supervises and counsels the patient and family regarding home nursing procedures and other care needs as appropriate to the patient’s condition.
• Exhibits critical thinking and applies them for continuous improvement of services and the agency
• Providing oversight of all patient care services and personnel, including supervising, guiding and directing staff.

Home Health Assistant Clinical Manager Requirements:

• Active Michigan Registered Nurse (RN) license
• Reliable transportation
• Valid driver's license and auto insurance in your name as a driver
• Home health care experience preferred
