ProCare LTC Pharmacy is seeking a Full Time Pharmacist for our Worthington, OH location.
A Pharmacist daily function is to review, enter, and clarify nurse/ MD questions regarding prescriptions for residents of a long-term care facility. Prepares medications by reviewing and interpreting physician orders and detecting therapeutic incompatibilities. Dispenses medications by compounding, packaging, and labeling pharmaceuticals. Controls medications by monitoring drug therapies; advising interventions.
Salary range: $55-65 an hour
Schedule : Rotating shifts with weekend and Holiday Rotation

Job Summary:
A Pharmacist function is to handle the day-to-day operations of a Long Term Care Pharmacy. Daily functions include, but are not limited to: prescription review, entering prescriptions, speaking with facilities regarding medication, ( documenting all communications regarding medications therapy ) and reviewing order entry work.
The practice of the profession of pharmacy is defined as the administering, preparing, compounding, preserving, or the dispensing of drugs, medicines and therapeutic devices on the basis of prescriptions or other legal authority, and collaborative drug therapy management in accordance with state and federal regulations.
Job Functions:

1. Enter and review faxed, verbal, electronic, and paper orders.
2. Initial Review orders (PV1) and Final Review Orders (PV2).
3. Contact physicians for prescriptions when appropriate.
4. Speak with providers regarding prescription issues and clarifications.
5. Enter, check, and replace contents of EKits and Automated Dispensing Machines (ADS).
6. Pull and log controlled substances.
7. Process new and completed backorders.
8. Enter and compound IV/TPN and non-sterile compounds.
9. Make necessary NDC changes.
10. Check pre-packed medications.
11. Supervise other pharmacists (act as PIC) and oversee Order Entry staff.
12. Process OOP/LOA requests as they come in.
13. Triage documents to the appropriate queue.
14. Complete Quality Assurance (QA) sheets as necessary.
15. Process orders for REMS medications (i.e. clozapine).
16. Making clinical decisions and performing clinical reviews of patient profiles.
17. Perform on-call duties as scheduled.
18. Knowledge of medications, interactions, and dosage.
19. Report medication errors as they occur
20. Receive and enter 222 forms from facilities.
21. Understand billing procedures and communicate with the billing department.
22. Perform pharmacy opening and closing functions.

Prior experience in long term care and with Frameworks and Docutrack information systems is strongly preferred .
