Clarest Health is looking to hire a LTC Prior Authorization Pharmacist to work in our brand-new, state-of-the-art Clinical Hub located in Phoenix, Arizona.

Available Schedules: Generally regular business hours with some rotating weekends required

About the role….

Daily functions include but is not limited to assessing the prescribed medication against coverage criteria, navigate pharmacy and electronic medical records systems to review a patient profile, and provide drug therapy recommendations to the facility. Some coverage to departments other than the Clinical Adjudication Center may be required as needed. The ideal candidate will be responsible and professional. Must be able to multi-task and work well in a fast-paced environment. Punctuality and good attendance are a requirement.

Key Responsibilities:

• Prepare prior authorizations for Long Term Care patients and providers
• Review prior authorization paperwork for accuracy and completeness
• Utilize clinical knowledge and insurance coverage criteria to make patient specific drug therapy recommendations
• Advise and guide other health care professionals including technicians, nurses, and providers on the medication coverage process, including appeals
• Other responsibilities, including prescription entry and verification, as assigned

Qualifications:

• Knowledge of pharmacy adjudication processes
• Knowledge of Medicare / Medicaid / Commercial insurance protocols preferred
• Ability to parse patient medical records for pertinent clinical rationale
• Familiarity with EMAR systems – PCC/MatrixCare experience a plus
• Familiarity with Framework and Docutrack a plus
