Job Summary

We are seeking self-disciplined and out-going professionals with an innate sense of urgency. TFC Case Coordinators are an integral part of the Treatment Foster Care team whose primary role is to support both the treatment foster child and the treatment foster family. Case management is performed in the spirit of the Wraparound Planning Model of care coordination and is rooted in strengths-based assessment and intervention

The ideal candidate will be self-disciplined, able to easily pivot in the face of crisis situations, bring a since of duty and will value getting it right. In addition to direct contact with our parents and the children in their home, the position is responsible for documentation and adherence to and enforcement of predetermined protocols, policies, and procedures with a high degree of accuracy.

Successful candidates must have a basic understanding of a specialized area within a comprehensive field of knowledge normally acquired through attainment of a Bachelor’s degree.

GeneralResponsibilities

  • Oversight to ensure compliance with state licensing/monitoring standards and CK Family Services mission, policies, and procedures
  • Support to promote placement stability (including crisis support)
  • Ongoing education/encouragement to maintain quality of care and prevent compassion fatigue
  • Information and linking for community resources to strengthen the family’s support system and meet the needs of the child
  • Assistance navigating and understanding the foster care system so that parents can do their part to best advocate for the children in their care
  • Consistent modeling and education regarding the effects of trauma on children with severe mental, emotional, or behavioral health needs
  • Facilitation and guidance through the Wraparound Planning Process to ensure the child’s underlying needs and voice are being prioritized in the permanency planning process

Children

  • Accurately and timely complete all paperwork required to manage a child’s care in the foster home according to Minimum Standards, contract standards, and company policy/procedure.
  • Regularly review all documentation provided by foster/adoptive parents, outside agencies and all other documentation relevant to assigned children and families upon receipt. Make immediate written request for missing or necessary documentation. Follow up until obtained. Immediately notify supervisor of issues.
  • Complete and turn in documentation to supervisor consistent with CM task list.
  • Regularly oversee child’s progress and make regular reports to supervisor.
  • Effectively coordinate provision of all clinical support services for children. Collect, review, and file all documentation relevant to those services.
  • Interview all children privately regarding satisfaction with placement, foster family and home situation. Immediately notify supervisor of issues.
  • Attend all PCs, court hearings, family group conferences, and Child & Family Team Meetings as required.
  • Attend all psychiatric appointments as required.
  • Act as a liaison with school personnel, children and foster/adoptive parents. Attend all ARDs and other school meetings as required.
  • Attend weekly TFC Treatment Team meetings.
  • Participate in pre-placement discussions and/visits with family and in coordination with other TFC staff when applicable. Follow-up with families regarding pre-placement visits.
  • Coordinate the establishment of needed services for each child prior to their placement into the home.
  • Work in close coordination with other professionals providing services to children in the home to ensure the best possible outcomes.
  • Participate in pre-service and ongoing training of TFC families, as well as development of training curriculum, as necessary.
  • Participate in support groups for TFC homes, when necessary.
  • Report suspected child abuse or neglect, immediately to the Abuse and Neglect Hotline and Case Management Supervisor.
  • Effectively communicate with DFPS regarding the child.
  • Thoroughly and timely complete any needed investigations as required.
  • Maintain regular and effective communication with supervisor about all incidents and report such immediately.

Foster/Adoptive Family

  • Effectively manage TFC homes and monitor family for compliance with all required regulations making regular reports to supervisor.
  • Maintain regular and effective communication with the TFC family, keep them informed and hold them accountable for the child’s service plan requirements.
  • Make visits to TFC homes as dictated by TFC team, no less than twice a month, to observe family interactions and the home environment. Immediately notify supervisor of any issues.
  • Consistently empower TFC parents through teaching and encouraging the use of therapeutic and trauma sensitive behavior intervention strategies in the home.
  • Consistently teach and train TFC parents in clinical parenting skills, organizational skills, advocacy, professionalism and trauma informed care.
  • Routinely teach and train TFC parents the appropriate use and completion of agency forms.
  • Provide regular verbal and written feedback/counsel to the TFC family regarding progress towards stability, identified needs, and any ongoing barriers.
  • Support the TFC family by serving as a liaison to DFPS staff regarding any and all child related needs or concerns.

Wraparound Services

  • Actively use the engagement and team preparation skills throughout all phases of the Wraparound process to engage the family and various team members.
  • Communicate and operationalize the four key elements throughout the Wraparound process: Determined by Family; Strengths Perspective; Driven by underlying needs; and Supported by an Effective Team.
  • Actively plan and facilitate Child and Family Team Meetings, including the ability to handle and resolve conflict.
  • Starting with the initial Child and Family Team meeting, obtain regular updates from the child’s Managing Conservator regarding permanency planning and progress.
  • Oversees the Wraparound process and monitors progress by following up with each family and team member in between CFT meetings.
  • Schedule and facilitate all Child and Family Team Meetings (CFTM) every 30 days.
  • Develop a crisis/safety plan upon initial meeting with the family and child. Continue to monitor and update the plan as needed based on the individualized needs of the foster child and the foster family.
  • Develop a transition plan with the Wraparound Team that is in line with the child’s permanency plan and ensures their needs are being met as they discharge from the TFC program.

General

  • Serve in the TFC On Call rotation for after hours and weekend coverage and support to TFC foster families.
  • Provide crisis management to families as assigned implementing clinical strategies and trauma informed care.
  • Attend trainings as scheduled and renew required training as needed.
  • Participate in meetings and chart audits as scheduled.
  • Complete and report documentation timely and accurately.
  • Open and process mail / e-mail daily in a timely manner.
  • Answer phone/text communication, voicemail, and respond to requests timely and accurately.
  • Input data into computer accurately and timely.
  • Represent CK Family Services in the community by attending meetings and gatherings as required.
  • Assist in staff and foster/adoption parent training as required.
  • Attend all clinical meetings and department meetings as scheduled.
  • Advocate with community partners for foster/adoptive children, parents, and CK staff.
  • Maintain compliance with all CK Family Services policies, procedures, and regulatory requirements including all state and federal laws
  • Review and supervise maintenance of complete and current records on each child and family in compliance with Minimum Standards and Contracting.
  • Maintain confidentiality as required by government law and regulations.
  • Perform other duties and tasks as requested.

Qualifications


Qualifications

  • Bachelor’s degree in Social Work or related field.
  • Minimum of three (3) years’ experience preferred.
  • Work deals mostly with areas such as preparing and reading data and figures and visual inspection involving small details. Visual work is primarily close to the observer. Depth perception and field of vision (peripheral) are important but not as critical as the ability to distinguish small details and markings very near to the observer.
  • Excellent organizational and follow-up skills.
  • Excellent interpersonal skills and demonstrated ability to effectively communicate orally and in writing with a variety of people; maintain confidentiality
  • Ability to be on-call as necessary and respond immediately to on call pager or phone.
  • Must have reliable transportation.
  • Ability to handle multiple tasks; self-starter; energetic; detail oriented, with excellent organizational skills.
  • Good character, personal attributes and mannerisms to be a good adult role model for residents and their families, and as a professional for case managers under supervision.
  • Ability work effectively under stress and remain flexible in changing situations.
  • Proficient in basic computer skills that include but are not limited to data entry, MS Office, jobsite portals and company software.
  • Ability to relate to multiples ideas and the concepts/theories behind the ideas; ability to remember multiple verbal and written tasks/assignments over several days and occasionally up to several months.
  • Perform the physical demands of the position
    • Ability to use up to 100 pounds of force occasionally, up to 20 pounds of force frequently, and/or up to 10 pounds of force continuously to move objects and/or people;
    • Ability to walk frequently; stand or sit for prolonged periods of time; squat occasionally, climb stairs infrequently, and kneel and twist as needed.
    • Sufficient good health to properly fulfill all role responsibilities. Employees shall not be permitted to work who have infectious diseases or skin lesions for the duration of the communicability.
    • Ability to perform moderately difficult manipulation such as writing, typing, foot and hand-eye coordination required.
