Description

We are searching for a self-disciplined, well-coordinated, and diplomatic professional. to join the CK Family.

As a Case Management Supervisor position is a clinical position that will require the successful candidate to develop and maintain functional, supportive relationships with a variety of professionals and with families who are seeking to care for children in the foster care system and/or who are seeking to adopt children. This position is responsible for the daily operation of client case management including: providing supervision to case managers, monitoring the progress of the children in care, support/guidance, and training for staff and adoptive/foster parents.

GeneralResponsibilities

  • Provide effective oversight of Case Managers to ensure they are working efficiently and effectively. This includes crisis management, timely submission of required paperwork and adequate support provided to their assigned families.
  • Timely and appropriately process serious incidents with case manager. If necessary, develop and implement an intervention/safety plan for the child and/or family. Notify Director of implementation of any established plans. Report any Serious Incidents, including suspected child abuse or neglect, immediately to the Abuse and Neglect Hotline, CPS and Director.
  • Ensure that serious incidents are reported and documented in the required time frames as specified in the Minimum Standards Table Subchapter D and Contracting Requirements. Follow up until completed document is filed within 30 days of incident.
  • Monthly or quarterly review of established safety plans to ensure continued compliance and appropriateness of risk reduction plan.
  • Conduct internal investigations as needed or directed.
  • Monitor and ensure child and family records are current according to CK Policy through database dashboards. Any non-compliances will have action plans identified and records current within the next 30 days. Any records more than 2 months behind will be discussed with Manager to develop a Performance Improvement Plan.
  • Participate in Treatment Team meetings for children’s Individual Services Plans as required.
  • Ensure appropriate and timely development of Individual Service Plans and Service Plan Addendums for children. Ensure plans are routed according to agency policy.
  • Monthly review and filing of child case notes to ensure accuracy and compliance per agency policy.
  • Quarterly review of foster home reviews for accuracy, thoroughness and documented per agency policy. Ensure final document is corrected and filed by the end of month submitted.
  • Timely completion of developmental and deficiency plans for families as needed. Ensure plan is completed and signed by family within 30 days after non-compliance date.
  • Complete all other necessary documentation timely and accurately.
  • Attend home visits per agency policy to assess and observe Case Manager’s clinical assessment skills adherence to CK home visit requirements and provide support to Case Managers and families as needed.
  • Attend community and CK trainings to enhance clinical and professional growth and development.
  • Represent and attend community meetings as required.
  • Provide professional growth and development to Case Managers to enhance skills in supporting families working with traumatized children. This includes Case Manager attendance at community and CK professional trainings as well as one on one coaching by Case Management Supervisor.
  • Attend Supervisor meetings as scheduled and required. Actively participate in data collection and reporting. Assist team members with action plans.
  • Participate in Quality Audits as scheduled. Ensure indicated correction of charts are completed within agency guidelines.
  • Participation in review and correction of identified charts for scheduled/unscheduled audits by outside agencies such as YFT, SSCC’s, Contracting and Licensing.
  • Effectively provide support to foster/adoptive parents when Case Managers are not in the office or when a higher-level consultation is required.
  • Conduct individual Case Manager Conferences in accordance with CK policy. Ensure form is completed thoroughly to document strengths and areas of growth. Ensure annual goals and objectives are reviewed at least on a quarterly basis. Forward completed Conference Form to Human Resources after conference is completed.
  • Conduct annual performance evaluations. Develop and communicate annual goals with Case Manager. Monitor and provide feedback regarding progress on goals in accordance with CK policy.
  • Attend and facilitate staff and foster/adoptive parent training and development as required.
  • Maintain confidentiality as required by government law and regulations, as well as maintain compliance with all Covenant Kids policies, procedures, and state and federal laws.
  • Maintain high standards of professional, technical, personal conduct and be a role model for others in the office.
  • Perform all other duties and tasks as requested.

Qualifications


  • Bachelor’s degree
  • Work deals mostly with areas such as preparing and reading data and figures and visual inspection involving small details. Visual work is primarily close to the observer. Depth perception and field of vision (peripheral) are important but not as critical as the ability to distinguish small details and markings very near to the observer.
  • Excellent organizational and follow-up skills.
  • Excellent interpersonal skills and demonstrated ability to effectively communicate orally and in writing with a variety of people; maintain confidentiality
  • Ability to motivate people; ability to develop and exercise good supervisory skills.
  • Ability to participate in training and facilitate training as required.
  • Ability to handle multiple tasks; self-starter; energetic; detail oriented.
  • Good character, personal attributes and mannerisms to be a good adult role model for residents and their families, and as a professional for case managers under supervision.
  • Ability work effectively under stress and remain flexible in changing situations.
  • Proficient in basic computer skills that include but are not limited to data entry, MS Office, jobsite portals and company software.
  • Ability to relate to multiples ideas and the concepts/theories behind the ideas; ability to remember multiple verbal and written tasks/assignments over several days and occasionally up to several months.
  • Perform the physical demands of the position
    • Ability to use up to 100 pounds of force occasionally, up to 20 pounds of force frequently, and/or up to 10 pounds of force continuously to move objects and/or people;
    • Ability to walk frequently; stand or sit for prolonged periods of time; squat occasionally, climb stairs infrequently, and kneel and twist as needed.
    • Sufficient good health to properly fulfill all role responsibilities. Employees shall not be permitted to work who have infectious diseases or skin lesions for the duration of the communicability.
    • Ability to perform moderately difficult manipulation such as writing, typing, foot and hand-eye coordination required.
