Job Summary

We are searching for a self-disciplined, passionate team player who thrives in a detail oriented, dynamic environment to join the CK Family.

As a Behavioral Health Wraparound Team Lead you will be responsible for the provision of support and mentoring/coaching to wraparound care coordinators who are part of a behavioral health team. This includes initial support to develop necessary skills to achieve agency expectations and requirements, continued support to maintain the same, and targeted support for those who are experiencing challenges related to job performance and expectations. The Wraparound Team Lead will be fully invested in the mission and methods of CKBH and will consistently present a positive message to those being mentored. The Behavioral Health Wraparound Team Lead will also perform all of the basic duties of the wraparound care coordinators they are expected to support. The Wraparound Team Lead will be well informed with the NWIC model to help grow and maintain the organization and those involved for high quality and high-fidelity practice and trained in Motivational Interviewing.

GeneralResponsibilities

  • Approve notes written by wraparound care coordinators within your team.
  • Assist the Wraparound Supervisor to support team members as assigned or directed.
  • Provide support as needed to wraparound care coordinators with routine or at-risk placements, including but not limited to: modeling appropriate techniques, brainstorming alternative strategies, accessing needed resources, assisting in the development and implementation of action plans, and assessing understanding and implementation of modeled services. Support will be given either face-to-face, by phone, or through email in a timely manner
  • Review the documentation of assigned wraparound care coordinators in the medical records data base as needed to provide coaching support and guidance.
  • Support assigned wraparound care coordinators to fully understand and complete all documents required by their work position, to include: initial intake documents, progress notes, behavioral questionnaires, recovery plans, CANS assessments, service request forms, safety plans, critical incident reports, case closures, psychiatric hospitalizations, move/people changes, time off requests, mileage documentation, expense reports and travel requests as needed.
  • Teach wraparound care coordinators that services delivered are to be consistent with family plan of care goals and measurable outcomes and are to be delivered in a professional manner.
  • Coach wraparound care coordinators to monitor their schedules and caseloads to ensure that all clients are being seen regularly and in a timely manner.
  • Assist assigned wraparound care coordinators in the development of skills needed to meet documentation clinical quality standards and Texas Administrative Code requirements.
  • Reinforce the expectation that services provided to clients by team members are to be delivered according to fidelity and that all services provided are to be authorized.
  • Support assigned wraparound care coordinators until they fully understand the use of all agency electronic systems utilized by the team.
  • Ensure personal and team compliance with all rules and regulations of DSHS, including licensure rules and regulations and suspected fraudulent practices.
  • Conduct home visits with assigned wraparound care coordinators as needed to provide initial support and instruction, or to strengthen wraparound care coordinator skills where indicated or directed by the Supervisor.
  • Make necessary recommendations related to assigned Wraparound care coordinator’s strengths, areas of needed growth, and goals to the Supervisor on a monthly basis or as needed. If the Wraparound care coordinator is on a Performance Improvement Plan this report will be attached to the PIP and included in the individual’s personnel record.
  • Participate in Treatment Team meetings when applicable and beneficial to the action plan for children and families.
  • Meet routinely with Wraparound Supervisor to discuss ongoing team strategies, agency planning, challenges or concerns related to assigned wraparound care coordinators, children/family needs or concerns, scheduling or caseload assignments, and other administrative needs or concerns of the team or agency.
  • Demonstrate problem solving skills, effective communication and responsible use of resources.
  • Attend and participate in scheduled training seminars and on-going general team meetings.
  • Maintain open communication with Case Managers, DFPS workers, therapists, and other collaterals within assigned homes.
  • Identify, secure and coordinate additional services or resources as needed for team members, families or children.
  • Open and process mail/e-mail daily. Ensure response and/or follow up is completed within one business day.
  • Answer phone, collect messages and respond to requests as received.
  • Input data into computer accurately and timely.
  • Represent CK Family Services in the community by attending meetings as scheduled or assigned.
  • Maintain confidentiality as required by government law and regulations, as well as maintain compliance with all CK Family Services policies, procedures, and state and federal laws
  • Maintain high standards of professional, technical, personal conduct and be a role model for others on the team.
  • Complete Serious Incidents Reports within 24 hours and forward to supervisor for review and assist team members in doing the same.
  • Report suspected child abuse or neglect immediately but no more than 24 hours to the Abuse and Neglect Hotline and Supervisor.
  • Maintain compliance with all CK Family Services policies/procedures, and regulatory compliance, to include all state and federal laws.

Qualifications


Qualifications

  • Degree in Counseling, Psychology, Social Work or related field as specified in the Texas Administrative Code for Wraparound Care Coordinator or Qualified Mental Health Professional (QMHP) qualifications
  • Must possess basic therapeutic skills.
  • The ability to incentivize people and the ability to develop and exercise good supervisory skills.
  • Experience and training in working with individuals with severs and persistent mental illness, children with severe emotional disturbances, and delivery of services based on curriculum.
  • Excellent organizational and follow-up skills.
  • Proficient in basic computer skills that include but are not limited to data entry, MS Office, jobsite portals and company software.
  • Must have reliable transportation.
  • Excellent interpersonal skills and demonstrated ability to effectively communicate with a variety of people
  • Must be comfortable with heavy travel to homes across the state of Texas.
  • Strong cognitive, decision-making and problem-solving skills and a willingness to take on new projects with a can do attitude
  • Strong desire to work as a team with a results-driven approach.
  • Demonstrated ability to act with integrity, professionalism, and confidentiality.
  • Ability work effectively under stress and remain flexible in changing situations.
  • Ability to be on-call as necessary and respond immediately to different communication platforms.
  • Ability to participate in training and facilitate training as required.
  • Perform the physical demands of the position
    • Ability to use up to 100 pounds of force occasionally, up to 20 pounds of force frequently, and/or up to 10 pounds of force continuously to move objects and/or people;
    • Ability to step backwards, grasp, bend at the waist and knees, lower 30-50 pounds to the ground, be able to get up and down from the floor without assistance, exhibit full range of motion in both arms and legs, and be able to do a deep, reverse lunge (SAMA Satori Alternatives to Managing Aggression).
    • Ability to walk frequently; stand or sit for prolonged periods of time; squat occasionally, climb stairs infrequently, and kneel and twist as needed.
    • Sufficient good health to properly fulfill all role responsibilities. Employees shall not be permitted to work who have infectious diseases or skin lesions for the duration of the communicability.
