Minimum Qualifications:
 Required:

  • High School Diploma or GED equivalent.
  • Must possess a valid Texas Driver’s license.
  • Must be certified by the Texas Department of State Health Services Community Health Worker (CHW) within 180 Days.

 Preferred:

  • Associate's Degree and Volunteering experience, working with community organizations or non-profits.

Position Overview: 
Under direct supervision, the Community Health Worker (CHW) I is responsible for helping community members and their families navigate and access community services, community resources, and adopt healthy behaviors. CHW is a core member of the Community and Clinical Health Bridge program aimed to improve obesity and related chronic disease prevention and care. 

 Essential Functions: • Provides culturally appropriate health education, information, and outreach in community-based settings (e.g., nutrition, physical activity, weight management and disease self-management education).
 • Establishes and maintains trusting relationships with community members and their families.
 • Manages assigned caseload of community members by maintaining regular communication and follow-up protocols
 • Documents all client encounters and contacts made on behalf of clients; completes and submits monthly reports; maintains comprehensive client files, which include client notes, release of information, assessments, and other documents.
