The City of Virginia Beach Department of Human Services is seeking to hire a candidate who is licensed as a QMHP-A and has experience working with clients with Substance Use Disorders (SUD) and care coordination services to include discharge services for the Drop In Center and IOP programs at the Pathways Center.

Responsibilities:

• Monitor individuals in the Drop In and IOP programs.
• Provide clinical documentation and assure that all filling of clinical information is done correctly and within the time frame required.
• Assists QA in the screening of client charting and reports all errors for corrective action with all errors found and corrected at 100% accuracy.
• Perform necessary care coordination functions related to service referrals, coordination of care, monitoring/follow-up, and case documentation in accordance with quality assurance and professional client standards.
• Collaborate as a member of a multidisciplinary team in order to assist individuals in recovery.
• Answer telephone inquiries.
• Work with community stakeholders.
• Provide referral sources.
• Monitor meal delivery.
• Drive individuals as needed.
• Attend treatment team and staff meetings as scheduled.
