Job Description

NYCHAs Department of Resident Services, Partnerships, and Initiatives (RSPI) seeks an experienced Social Worker to support its Family Partnerships Team. Family Partnerships staff respond to emergent resident social and mental health crises, coordinating care responses with partner NYCHA departments, Resident Leaders and external social service agencies and organizations. Staff also manage short-term client cases, connecting these at-need residents with critical health programs, social services, and support initiatives in the areas of household stability, youth and older-adult social engagement, economic opportunity, and educational programming.

Under general supervision of the Family Partnerships Social Work Supervisor II, and with mid-latitude for independent initiative and judgment, responsibilities of the Social Worker role include, but not limited to the following:

1. Maintain a short- and mid-term client caseload with detailed documentation, case notes and regular reporting.

2. Develop plans to provide residents with appropriate social services; employ an early intervention, preventive focus, intensive case management approach.

3. Travel to individual and joint home visits across the borough several days each week.

4. Conduct in-person and video assessments of residents' social service needs and coordinate those services with internal and external stakeholders.

5. Conduct Psycho-Social Assessments/GAL Evaluations to determine competency.

6. Track all resident and resident-related interactions through a digital case management system, prepare and maintain case records and written reports, and pull reports and data upon request.

7. Communicate regularly with NYCHAs Property Management, Law, Application and Tenancy, Skilled Trades, Safety and Security, Healthy Homes, and Finance departments to address co-existing resident risk factors.

8. Coordinate regularly with external social services partners and providers, including Adult Protective Services (APS), Administration for Child Services (ACS), Department of Social Services (DSS), Human Resource Administration (HRA) and a variety of Community-based Organizations (CBOs).

9. Report all cases of suspected child abuse and/or neglect to New York State Central Register.

10. Work on a Team of Social Service Supervisors, Coordinators and Associates to coordinate consistent service delivery and feedback for residents under care.

11. Identify gaps in residents social service needs and then partner with sister departments in RSPI, and external social services providers to creatively fill/solve those needs.

12. Represent NYCHA at resident, community, and citywide events.

13. Utilize Microsoft Office suite of applications and (Teams, Word, Excel, PowerPoint, Visio, and Outlook) as well as video conferencing platforms and task management tools.

14. Contribute to improved outreach, referral, and recruitment procedure updates as needed.

15. Respond to emergent social services needs and escalations as needed.

16. Attend central office and off sites meeting as required.

17. Perform additional related duties incidental to the work listed above as needed.

Qualifications

Qualification Requirements

A Master's Degree in Social Work from an accredited school of social work.

License Requirements

You will be required to have a valid Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) license issued by the New York State Department of Education within one year of the date of appointment. This license must be presented to the appointing officer at the time of appointment or, if it is obtained after appointment, at the time it is received. This license must be maintained for the duration of your employment.

If you fail to obtain your LMSW or LCSW license within one year of the date of appointment, your probationary period will be automatically extended for six months. If you fail to obtain the required license by the end of 18 months of service, you will be terminated.

Special Note

Section 424-a of the New York Social service Law requires an authorized agency to inquire whether a candidate for employment with child-care responsibilities has been the subject of a child abuse and maltreatment report. The agency has the discretion to assign a candidate who has been the subject of a child abuse and maltreatment report to a position with no child-care responsibilities.
