Community Health Worker- Contract

Division: Gary Health DepartmentReports to: Health Services Manager 

Status: Contract Position Salary: $15.00 ( 24 Hours/Week)

Promote health equity through education and building relationships with other organizations within the community. Responsible for developing and attending community outreach events. This is a part-time position, 24 hrs./week. May be required to work weekends and or after normal working hours.

• High School graduate or equivalent 
• Experience in Public Health and outreach process preferred
• Must have excellent customer service skills
