Provide professional and social work services specifically focused on the needs of individuals, families, people with disabilities, the elderly and their caregivers.

• Perform assessment of client’s needs and make appropriate referrals based on assessment (for example: Operation Hope Food Pantry, Fairfield Housing Authority, Center for Women and Families, Dress for Success, Legal Aid, CT Works, etc.).
• Therapeutic support to clients, many whom have medical, developmental and psychiatric disabilities.
• Make home visits to individuals unable to come to the office due to a disability, age or lack of transportation.
• Case management activities for numerous clients.
• Assist clients with State of CT Department of Social Services entitlement application for Medicaid, HUSKY, SNAP (food stamps) and/or cash benefits.
• Advocate for Fairfield citizens with various agencies, such as State of CT Department of Social Services.
• Assist clients navigating various federal agencies, such as the Social Security Administration. Provide information on federal resources available.
• Collaborate with other Town of Fairfield departments as needed, such as the Health Department, Community and Economic Development, Tax Assessor, Board of Education, Parks and Recreation and Public works.
• Respond to outreach referrals from the Fire Department and Police Department.
• Provide information on mental health resources, including counseling centers, support groups, and Emergency Mobile Crisis Services.
• Perform CHOICES Medicare Counseling as needed.
• Serve as the contact for State of CT Marshals for evictions within the Town of Fairfield and follow Town of Fairfield eviction procedures.
• Maintain awareness of resources for eviction and foreclosure prevention.
• Provide resources for subsidized housing, including Section 8 and elderly and disabled housing.
• Refer clients to programs for employment training, support and placement.

• Act as a mandated reporter to State of CT Department of Children and Families for any concerns of abuse or neglect of children.
• Act as a mandated reporter to the State of CT Protective Services for the Elderly for any concerns of abuse or neglect of individuals over the age of 60.
• Maintain appropriate case notes in client files.
• Contact utility company liaisons (United Illuminating and Southern CT Gas) on behalf of clients for shut-off notices, payment plan arrangements and Winter Protection.
• Complete application for ABCD Energy Assistance.
• Refer clients to other sources of heating assistance programs (Operation Fuel, Salvation Army).
• Work with the Salvation Army Southern New England Chapter for grant assistance and back to school backpack and shoe program. Complete necessary paperwork for client assistance to access funding.
• Publicize and refer clients to Salvation Army Camp Connri Summer Camp.
• Submit press releases to local media for Energy Assistance, Salvation Army Camp Connri, Back to School Supplies Drive.
• Organize annual Holiday Giving Programs, Campership and Back to Schools Programs.
• Accept referrals from and collaborate with Fairfield School Social Workers.
• Provide information and applications on transportation resources, including the Dial-a-ride program.
• Answer phone calls and office tasks as needed.
• Review and discuss with other department members financial requests for assistance for utilities, oil and other requests.
• Refer clients to West Bridge Coalition or other handyman homeowner repair assistance programs.
